Mission & Impact Statements Mission Last Chance Pet Rescue, Inc. was formed to provide exposure and rescue for the animals held by animal control facilities in Dickson County, Tennessee and to locate affordable spay/neuter options for individually owned companion animals. Background The experience of our founding board members is extensive. We saw the need to help the animals held by an animal control facility that does not have access to the internet to post available animals, is not open to the public and with no access to a low cost spay/neuter clinic in Dickson TN, so we formed Last Chance Pet Rescue, Inc. It is our goal to save as many animals as possible and to break the chain of unwanted animals being born. Impact In 2013, while continuing to save animals from animal control facilites weekly, we received our first spay/neuter grant from the State of TN. We received a grant from the State of TN in 2014 and have been approved in 2015 for a $5000.00 grant from the State for use in 2016. We received a grant from The Community Foundation of Middle TN for use in 2015 and we have already altered close to 130 community animals. We wish to expand our spay/neuter program as we are the ONLY rescue in Dickson County offering spay/neuter assistance to private individuals. Needs Funds to provide food and medical care for our "Sanctuary Animals". Animals that have been in long term foster care and have been deemed unadoptable either due to medical or temperament issues1. Funding for our spay/neuter program for owned companion animals. 2. Funding for the medical needs of our foster animals. 3. Foster homes for the animals we can rescue from animal control facilities. 4. Donations of cat/kitten food, dog/puppy food, blankets, cleaning supplies, etc. 5. Volunteers to help with our foster animals and our events.6. Funds to provide food and medical care for our "Sanctuary Animals", animals that have been in long term foster care and have been deemed unadoptable due to medical or temperament issues. Other ways to donate, support, or volunteer We accept donations via Paypal (lastchancepetrescue-dickson@yahoo.com), on our Facebook site, and by mail (PO Box 118, White Bluff TN 37187). Donations can also be made directly to the veterinary clinics that treat our animals. Board Chair Statement I was on the board of directors for our local Humane Society for over 10 years. When that entity went from an "open door" facility to a "no-kill, owner surrender animals only" entity, I could not live with the fact that the animals held just down the street at animal control would no longer get a chance at being adopted. I left that organization and began forming Last Chance Pet Rescue. There is also no low cost spay/neuter clinic within our county, so individuals that do not have the funds to have their animals altered are unable to prevent unwanted litters of puppies and kittens. This horrible cycle must end. The fewer unwanted animals being born, the fewer animals ending up at animal control. CEO Statement Last Chance Pet Rescue, Inc was formed to lend a voice to the animals held by animal control facilities in Dickson County TN. The main facility is not open to the public, does not have access to the internet so they can be viewed by the public and has no formal adoption process. We are the only rescue in Dickson TN that was formed for these animals. There is no formal spay/neuter assistance in Dickson County TN either. We are the only group in Dickson County assisting private indviduals to spay/neuter their companion animals. We want to expand this program to help more families. 3

Programs Programs Spay/neuter assistance program. Description It is the goal of this program to offer financial assistance to individuals to have their pets spayed/neutered. We have a working relationship with a veterinary clinic in Dickson TN to handle these surgeries. This will be an ongoing program. Budget 8000 Category Animal-Related, General/Other Animal-Related, General/Other Population Served General/Unspecified,, Short Term Success Long term Success The short-term success is the same as the loan-term success. Stopping the birth of unwanted kittens and puppies. The ultimate goal for this program is spay/neuter as many owned animals as possible. This will result in fewer unwanted puppies and kittens being born that either are abandoned, turned over to animal control or rescue groups. You cannot adopt your way out of companion pet overpopulation. Spay/neuter is the only answer Program Success Monitored By We have already begun to see fewer puppies and kittens being taken in by our animal control facility. Examples of Program Success The owners of the animals that are spay/neutered due to our program are so grateful. They cannot afford the surgeries and do not want their pet to have unwanted litters. Every single animal that is spay/neutered is a success story! Sanctuary Animals Description We have many animals in long term foster care that have been deemed unadoptable due to medical issues or temperament issues. We will continue to care for these animals until they pass away from natural causes. These animals were rescued from animal control and deserve to be treated with care and love for the remainder of their lives. Budget 1500 Category Animal-Related, General/Other Animal Protection & Welfare Population Served,, Short Term Success Long term Success Program Success Monitored By Examples of Program Success The "Sanctuary Animals" were originally rescued from our local animal control facility. We saved their lives as this is a kill facility. The foster animals that are considered "Sanctuary Animals" will be cared for in their foster homes until they pass away. The "Sanctuary Animals" are cared for on a daily basis. Animals in our "Sanctuary Animals" program were saved from animal control and are living out their lives in a safe, home environment. To them, they are home and are loved. 4

Animal Control animal assistance Description We visit the animal control facility in Dickson TN every Saturday to photograph and assess the new animals picked up by the animal control officers the week before. We provide basic wound care and deworming for each animal. We check each animal for a microchip. We then edit and post all the pictures on many adoption websites, our Facebook page and do a mass emailing to other rescues in Tennessee try to move the animals into rescues, adopted or returned to their owner. If an animal is very ill or wounded, we provide veterinary care as the city and county do not provide this for the animals. Most animals taken into animal control are either released to rescues, returned to owner or adopted from the facility. If we were not providing this service, these animals would have no exposure to the public or rescues. Long term Success When we first started visiting animal control over 7 years ago, they were euthanizing animals on almost a weekly basis. w they very rarely have to euthanize an adoptable animal. We will continue to perform these services for as long as we physically can. Examples of Program Success The animal control officers have gone from euthanizing on almost a weekly basis to very rarely at this point. Many animals have been returned to their owners or adopted to families. CEO Comments Funding for the spay/neuter program is the main challenge. The funds generally come from grant opportunities and private donations. All funds for this program go directly to pay for the actual surgeries. There are no overhead or payroll costs. We are a small group of dedicated individuals doing all we can to reduce the number of unwanted puppies and kittens being born in Dickson County. Funding for the medical and food needs for our Sanctuary Animal program is an ongoing struggle as well. We have committed to the care of these animals for the remainder of their lives. 5
