HOUSE RULES (because your dog means a lot to us) Everything we do here at Oh Hello Dog is intended to ensure that your dog has the best time in a safe environment. The following house rules allow us to care for your dog in the best way we can. Requirements 1. It s the responsibility of the dog s owner to bring in their dog s vaccination records from a licensed veterinarian at enrollment and when vaccines are renewed. Provided contact information, OHD is happy to acquire the forms directly from your veterinarian. The following is required for each dog: Proof of DHPP (Distemper, Hepatitis, Parvovirus and Parainfluenza) vaccination. Proof of Bordetella vaccination. Proof of Rabies vaccination. All shots must be administered at least seven (7) days prior to joining. Proof of Spay/Neuter for all dogs six (6) months or older. 2. Dogs must be at least five (5) months old and have full vaccine series administered at least one (1) month prior to joining. 3. Dogs must be in good health and free of parasites. If your dog has any symptoms of illness including but not limited to coughing, sneezing, wheezing, diarrhea, vomiting, lethargy, runny eyes/nose, please wait until your dog is no longer showing these symptoms before attending OHD. Dogs who have had a confirmed illness must provide health clearance from a vet. 4. Dogs must be on some type of flea and tick preventative. In the event fleas or ticks are found on a dog, a flea bath will be administered and the owner will assume associated charges. 5. Collars and I.D. tags are required on all dogs. Hours We are open Monday - Friday from 7:00am - 8:00pm and Saturday - Sunday from 8:00am - 7:00pm including holidays. We offer after hour pick-ups up to an hour after closing for a fee of $10.00. Please inquire for more details. Evaluation We would love to get to know your dog so that we can figure out what OHD can offer your dog and family. Upon registration, please call to schedule an evaluation. Fees Payment is due in full at pick up for most stays. For stays longer than one week, 50% of the bill is required at the time of drop off and 50% at pick up. Cash, check, and all major credit cards accepted. There is a $50.00 fee for all returned checks. Pre-paid packages are non-refundable and non-transferrable upon evaluation.

Daycare, Boarding & Private Suites 1. Cage-Free Daycare: Six (6) hours or less is considered a half day and more than six (6) hours is considered a full day. Play areas are toy and treat free zones. 2. Cage-Free Boarding: Your dog can check in whenever you like however we highly recommend dropping your dog off by 12:00pm so that your pup can run and play with their friends before their first night. Check out is at 12:00pm. If picked up after 12:00pm, OHD s standard half day daycare rate will apply. If you do not pick up by closing and have not made prior arrangements for an after hours pick up, your dog will automatically be boarded for another night and standard fees will apply. It is best to plan for boarding at least a couple weeks in advance in order to obtain services on the desired dates. We will do our best to accommodate last minute requests. We also require 24 hours notice for any boarding cancellations or a $50.00 fee will be incurred. During peak holiday periods, we may have a non-refundable deposit, minimum stay requirements and/or increased pricing. 3. Private Suites: We are able to accommodate a maximum of 2 dogs per private suite. For the evaluation, we will see if your dog is able to be in a room by him or herself, their walking manners and our handlers cues. Personal toys and/or belongings are allowed for dogs staying in our private suites. Food & Medication 1. Food: We recommend that you bring your dog s own food when boarding, but this is not required. Continuing on their own food helps them avoid digestion issues that can result from switching food. Feeding times occur at 6:00am, 12:00pm, and 6:00pm. Please pack your dog s food with their first and last name clearly labeled on it and instructions about how much and how often to feed your dog. If you do not bring your dog s own food, we are more than happy to feed your dog our house food at an additional cost please inquire for details. We also recommend bringing some extra food just in case your travel plans change. If your dog runs out of food, we will feed them our house food at your expense unless other arrangements are made. We have plenty of feeding bowls so it is not necessary to bring a bowl for your dog.

2. Medication: OHD does not charge an additional fee for administering medications. In order for us to be precise and thorough while giving your dog their medication(s), please ask our staff for our medication form. Furthermore, if your dog is taking any medications, please bring them in the original bottle and provide clear written instructions. This ensures that we can provide accurate information to a veterinarian in the event of an emergency. Baths All dogs who come in for a bath must be registered in our system as well as have proof of vaccination records. If you would like for your dog has to spend more hours at OHD beyond the grooming appointment window (2 hours), we can apply our standard half day or full day rate. General Information 1. Dogs must be on leash while entering and exiting the lobby as well as in the parking lot. 2. For security purposes, proper photo identification may be requested for dog release. 3. Please note that your dog cannot have any contagious diseases to attend OHD. If your dog is sick, we ask that you wait for at least thirty (30) days from full recovery before returning to daycare. 4. We love rescues (in fact our 3 shop dogs are all city shelter alumni!), however all shelter dogs must be out of the shelter system for at least thirty (30) days before enrolling at OHD. This allows them time to settle into their new environment before introducing them to a group play environment. 5. Feel free to drop by and chat with us. OH HELLO DOG WAIVER Disclosure: I hereby certify that I am the legal owner of. I understand and agree that in admitting my dog, Oh Hello Dog ( OHD ) has relied on my representation that my dog is in good health and has not harmed or shown aggressive or threatening behavior towards any person or dog. Assumption of Risk: I understand that I am solely responsible for any harm caused by my dog to any other dog, employee, guest [person] while my dog is attending daycare, boarding or washing at OHD. Furthermore, I understand that OHD will exercise due diligence and care in the guardianship of my pet. I hereby waive and release OHD and its employees and agents from any and all liability of any nature, for injury or damage, including that which may result from the action of any dog including my own, and I expressly assume the risk of such damage or injury while my dog participates in or attends any function of OHD, while on the grounds or in the surrounding areas. Although OHD will carefully screen all evaluations, we occasionally discover that this is not an appropriate environment for every dog. I understand that OHD reserves the right to permanently remove my dog from daycare, boarding or washing at any time for whatever reason. Health & Medical: I hereby certify that the above named dog has not been ill with any known contagious diseases and has fully been vaccinated for DHPP (distemper, hepatitis, parvovirus, and parainfluenza), as well as bordetella, and rabies in accordance with local laws. In the event of an injury, emergency, or when OHD, in its sole discretion, deems medical care is important to your dog, I authorize OHD to obtain medical attention for my dog. I grant OHD full power of decision making involving the medical treatment of my dog including transport and care to and from any agent. If the illness or injury requires medical attention, OHD will make every effort to contact you. I accept responsibility for any and all associated expenses. Abandonment: If your dog is not picked up on the scheduled check-out date and fourteen (14) calendar days have passed with no contact made, your dog will be considered abandoned. OHD will report abandoned dogs and take appropriate action if not contacted by you within the allowed time period. I understand that dog abandonment may be a criminal or civil violation of the statutes of the State of California. (Shame on me if I ever do this.) Media: Photos, videos or other digital recordings will be taken at the OHD facility of dogs, customers, and staff on a regular basis for use in marketing, promotional materials, and social media by OHD. I give OHD permission to use, reproduce and alter images in print or electronic format, including on the internet. We also promise to make your dog look super cute in any kind of media. What to Expect When You are New to Dog Daycare The Evaluation: Your dog s experience is very important to us. Your dog s evaluation will feel like a big playdate for her/him. We will slowly introduce your pup to our dogs and find out if he/she loves to be in a stimulating setting with many dogs with many different play-styles. Not all dogs are destined to love daycare -- and that is perfectly normal. Just like different people like socializing in different ways, it is the same with dogs. If we get the sense that your dog likes to play in small groups or like to relax by themselves and play games with the humans, then we ll talk it over with you and make a plan together. Getting Sick: Unfortunately, your dog may get sick during daycare... just like children at school. The standard annual vaccines are great for protecting your dogs against the more serious disease, but here are a couple to keep an eye out for: Kennel Cough -- This is the canine version of the flu, is not deadly, and is highly contagious. You might notice a deep sounding cough, white foamy mucus that gets coughed up, and listlessness. Parasites -- Fleas are not fun for dogs or humans. All dogs attending Oh Hello Dog are required to be on flea prevention. We do not allow flea collars as they may not safe for dogs who are playing with each other. Worms are also another parasite your dog may have. If we notice that a dog in our daycare has worms, the dog will be immediately isolated (in a lovely little room where he/she will get a lot of love) and his/her owner s will be contacted. Giardia -- This one will make your dog have diarrhea and vomit, and sometimes it might even appear bloody. While it s scary to see, your veterinarian will be able to treat it and most dogs recover quickly. Getting Dirty & Roughed Up: Dogs play with their mouths and teeth. If your dog is actively playing with the other dogs at our daycare, please expect that they will get scratches, nicks, scrapes, and even the occasional puncture wound. Our employees are outstanding at keeping an eye on the dogs and will make sure that they break up any play with even slightly negative energy. Nevertheless, your dog may come home with small injuries here and there. We promise to keep you appraised if anything serious comes up. Thirsty: In the beginning, dogs get distracted when they re at daycare. Things are new, exciting, and distracting, and they may forget to drink water -- even if there is water constantly available in multiple locations in all areas.
