HOW TO USE THE ENTRY BLANK Please note that Obedience and Rally are separate trials. Use one entry blank per dog if information is exactly the SAME for each day. Carefully check the block (blocks) for each trial in which you wish to enter your dog. You MUST use a SEPARATE entry blank if you are entering your dog in a different class on different days. When submitting entries do not use any delivery method that requires a signature. Doing so may delay receipt of your entry. EXHIBITOR INFORMATION Make all checks and money orders payable to: Hampton Roads Obedience Training Club, Inc. (HROTC). Trial Secretary assumes no responsibility for cash sent through the mail. Mail or deliver all entries for all trials with fees to: Beverly McGhghy, 185 Herndon Road, Virginia Beach, VA 23462. Entries without fees will not be accepted. Acknowledgement of entries will be made as soon as practical. Should such acknowledgement not be received by the exhibitor within a reasonable time, please notify the Beverly McGhghy, Trial Secretary. Entries not on official AKC entry forms or photocopies of entry forms without the agreement and rules on the reverse side of the official AKC entry form are not acceptable. Returned checks do not constitute a valid entry. A $25.00 service fee will be added for all returned checks. Express entries: Please use "release signature" or the entry will be delayed. No entry shall be made and no entry shall be accepted which specifies any conditions as to its acceptance. Females In Season: All but $3.00 of the entry fee may be refunded for bitches who come in season after the closing date by supplying the Trial Secretary with a certificate from your veterinarian dated on or before the day of the trial. Entry fees shall not be refunded if a dog is absent, disqualified, excused by Veterinarian or Judge, or barred from competition by action of the Trial Committee. If because of riots, civil disturbances or other acts beyond the control of the management it is impossible to open or to complete the show, no refund of entry fee will be made. Extreme weather conditions, such as, but not limited to snow storms, hurricanes, lightning, extreme heat, heavy rains, or other circumstances including, but not limited to, the condition of the facilities or grounds and/or the ingress and egress from the grounds, must be considered for the health and safety of the dogs, exhibitors and spectators. The well-being of dogs, exhibitors and spectators is of paramount importance and, in the event it is necessary to cancel or stop the show before completion, no refund of entry fee will be made. Consult judging program for time of judging. Dogs may arrive at any time prior to their scheduled time of judging. Dogs not required for further judging will be excused. All dogs present must be held on leash or confined to their crates except when being judged. Judges will not wait for any dog holding up a class. Owners or agents are responsible for the presence of their dogs in judging ring when their classes are called to be judged. Exhibitors should follow their veterinarians recommendation to assure their dogs are free of internal and external parasites, any communicable diseases, and have appropriate vaccinations. 6
