(ssigned by ) R # Survival Surgery/Procedure Form Important Notes: The regulations on the use of laboratory animals require specific documentation for all survival surgical procedures. ppropriate forms can be obtained from the omparative Medicine Department (ext# 31795/77672/24780). Some forms must be completed and returned prior to surgery while others are to be completed intraoperatively and after the recovery period. ll forms must be returned to the veterinarians. Please reserve the survival surgery suite well in advance and discuss any requests for support with the veterinarians. re any animals to remain alive for any period of time after a surgical procedure?
