Daycare & Boarding Policies Sex All dogs must be spayed or neutered by 7 months of age unless medically advised otherwise. We reserve the right to suspend any dog's membership until they are spayed or neutered if we feel being in tact is affecting their group experience or is a safety concern. Shots All dogs must be up-to-date on their Rabies vaccine, per the state legal standards (as of this date that means vaccination at 12 weeks, 1 year and then every 3 years). All other vaccines can be given per veterinarian s recommendations. Owners must provide a copy of current vaccinations or titers to The Good Dog Spot! at time of acceptance and as shots are updated. Health Only healthy dogs can come to daycare/boarding. Owners who observe signs of illness are to keep the dog home until the situation is clarified. Dogs who appear ill at daycare/boarding will be segregated or taken to the vet if needed. Dogs who appear ill while being boarded may be transferred, at owner s expense, to an overnight care facility at a veterinary clinic. Behavior All dogs must be non-aggressive to both people and dogs as judged by the interview questions asked by our trained staff and our ongoing observations. We will provide a verbal or written report of your dog s behavior when you pick up your dog and will advise you of any concerns at that time. We reserve the right to declare a dog s behavior as unfit for our daycare/boarding environment at any time, but we endeavor to inform you in advance of any ongoing concerns and to work with you on a solution when possible. Additionally, we recommend that all dogs attend a basic training course either here or elsewhere, for the safety of the dogs and for the enhancement of the dog-owner bond, as we are promoting and supporting the WHOLE dog. Application/Interview All dogs must be interviewed prior to attendance at daycare/boarding. All dogs must have an approved application on file. Waivers All dogs must have approved waivers on file, signed by the owner, in order to attend daycare/boarding. 2 of 7

Sign your dog up for this exclusive service and they will receive: A nightly tuck in treat, a daily leash walk (or cuddle time/private playtime depending on the weather!) with one of our team members, and a daily photo. Cat Boarding: $25 for the first cat, $15 for the second cat from the same household in the same condo. PLEASE NOTE: Pick up and drop off are not available week days from 12 1:30 pm while dogs are enjoying their quiet time. Boarding drop off and pick up is available on all holidays and weekends from 8 11 am and 3 5 pm. There is no extra charge for holiday services. Any dogs who are here beyond our scheduled closing time will be set up as a boarder with food, water and bedding and you will be charged the overnight boarding fee accordingly Dogs are always napped from 12:00 1:30 pm and after 5:30 pm. 3 of 7 Dogs who need to calm down or who seem overly anxious may receive quiet time on an as needed basis

Boarding Policies Boarding dogs are subject to the above daycare policies except as noted below: Rates The fee for overnight boarding is $41 (plus sales tax), which includes daycare on the first day through 12 pm on the day of pick up. You are charged based on arrival for boarding at 7 am regardless of the time at which you actually drop off your dog on their first day. We encourage you to give your dog the full day to play and tire themselves out, meet their friends, and acclimate to the new environment before they bed down for the night. We also highly recommend that your dog attend at least a half day of daycare prior to boarding so they can become familiar with our facility. If picked up before 12 pm there is no charge for that day. After 12 pm there is a $15 added fee per dog to cover the additional day of daycare. Health Dogs arriving for boarding with or developing (while boarding) signs of coughs, fevers, diarrhea or vomiting will be segregated and/or taken to the vet per judgment of the manager on duty. We reserve the right to take any boarding dog to see a veterinarian at our discretion, whether due to injury, illness, or general condition, with all expenses to be reimbursed to us by you, the dog s owner. Based on observed signs of illness or veterinary diagnosis, we reserve the right to decide to have a staff member sleep at the day care to look after your dog for an additional fee of $15 per night, or to have the dog transferred to a veterinary facility or some other legal boarding facility that is able to keep the dog isolated, instead of at our facility, with all expenses to be reimbursed to us by you, the dog s owner. You are exempt from paying boarding fees here when we arrange to have your dog boarded elsewhere, however, you may be charged up to $20 per day for our efforts at transporting and/or attending on your dog at the vet or other situation. Behavior Evaluation/Interview We need to evaluate boarding dogs for signs of stress when crated or when alone. We may request a daycare day or overnight stay as part of your dog s evaluation prior to accepting your dog for boarding; regular daycare and overnight rates apply. On Your First Day So it s your first day the The Good Dog Spot... What to do: Please enter the building with your dog under control. We ask that all dogs be leashed on no more than a 6 lead. We highly discourage retractable leashes but if one is being used we ask that you keep it locked at a maximum of 6. If a dog is already in the hallway preparing to enter daycare or to leave, please wait with your dog in the lobby on the outside of the white gates. Many dogs who are otherwise very social can be reactive on leash. Leaving space at pickup/drop off time can make the transition much easier and safer for everyone. If we don t see you enter please ring the door bell on the hallway wall next to the playroom window. We ask that you never come behind our hallway gate without an employee s permission as this is not a public area and we often will have dogs being moved about in the back of the building. When it is your turn to approach the drop off area you may remove your dog s collar and leash and place them in their designated cubby. We work hard to maintain good gate manners so we may ask your dog to sit nicely before we accept them through the gate. We encourage you to do the same. Once we let them through the gate they are on their way to play! Please feel free to observe dog play through the windows in our lobby (that s why we have them!), but we ask that you refrain from opening them to talk to your dog (or other dogs) or tapping on them to get attention. This creates a lot of excitement among the pack and can lead to negative behavior and stress for the group as well as your dog. As much fun as they have playing it s hard for them to focus on their friends when they see you separated from them! What to expect: If your dog is unaccustomed to an all day playgroup environment they will likely be very tired upon returning home. It is not unusual that they may have loose stool from all the excitement as well. Your dog has access to water all day while at The Good Dog Spot but will probably want a good long drink when they get home (think about going for an all day hike when you normally just take a 10 minute walk). This is normal. But if you notice lethargy lasting beyond 48 hours or lasting diarrhea, vomiting, etc it is always best to talk to your vet. 5 of 7

Important Information Accidents & Injuries When dogs play together things can get rough even when no aggression is involved. While our staff will do everything possible to avoid problems, it is not unlikely that your dog could experience the following as a result of being here: neck problems, torn and detached toe nails, minor cuts, scrapes and teeth marks, tooth loss or breakage, injured legs, tendons, joints etc. Beyond this, it is unlikely, but possible, that your dog could suffer serious injury from a dog fight. There is also a slight possibility that your dog could escape from our care. Fleas Ticks are also prevalent in most outdoor areas (your yard, the woods, etc.) We ask that all dogs attending daycare/boarding are treated with Frontline or Advantix. This will not only protect your dog while he/she is here, but will decrease the chances of your dog picking up fleas elsewhere and bringing them here. Dogs with an obvious flea infestation will be screened out as currently unhealthy. However, as you know, fleas are not always so obvious. The staff will inform owners if any fleas are noted on a dog so that they can begin to take care of the problem. Other health related concerns We endeavor to make daycare/boarding as clean and healthy as possible, nevertheless, your dog may be exposed to health issues. The staff provides an on-going visual assessment of the dogs apparent physical ability to handle daycare/ boarding activities and any observed concerns will be brought to the owner s attention. However, we are not able to provide professional assessment of any dog s actual physical state and therefore the owner is always responsible for knowing their dog s limitations and, by bringing the dog to daycare/boarding, is asserting his/her dog s ability to handle the physical play at daycare/boarding. Please remember that it is not uncommon for dogs to develop genetic structural issues, such as hip displaysia, between 10 months and 2 years of age and that the daycare/boarding activities are not the cause of their occurrence. Regardless of all screening, communicable diseases can be passed to and from vaccinated dogs, and vaccinations are not available for many of the things your dog may contract. Dog daycare/boarding is very like a child s daycare in passing around cold/flu-like symptoms several times a year. Dogs who have had the Bordatella (commonly called kennel cough ) vaccine can STILL CONTRACT Bordatella (a deep hacking cough). In the case of both of these illnesses, we have found that most dogs build an immunity after the first few go-rounds, while others (a very few of the total population) get something each and every time it goes around. An even smaller amount of our dogs at daycare/boarding get sicker than the other dogs each time. There are no obvious signs of ill-health on any of the dogs who tend to be less resistant and resilient than the others. We do not close the daycare for these symptoms, but we do request that any dog showing signs of any illness be kept home. Other potential health concerns may include, but are not limited to: skin contaminants, water contaminants, feces-borne contaminants, airborne contaminants. The staff will observe for any potential signs of illness and seek to control for it in as timely a manner as possible, but it is possible that your dog will require veterinary treatment for a condition possibly picked up from other dogs at daycare/boarding. The Good Dog Spot! accepts no responsibility or financial obligation for any such treatment your dog may require. 6 of 7

AGREEMENT & WAIVER 1. I understand and agree that my dog will be playing and socializing with other dogs that I do not know while at The Good Dog Spot! and I give permission to the staff to determine the viability of my dog and all other dogs to play together. 2. I understand and agree that I am fully responsible for any harm caused by my dog to another dog or person while at the center. 3. I understand that my dog may incur injury or illness while at daycare, I have been informed of the possibilities, and I understand and agree that The Good Dog Spot! is not liable for any health conditions, problems, accidents or harm that comes to my dog providing reasonable care and precautions are followed, especially those mentioned in the policies, and I hereby release from liability of any kind and agree to hold harmless The Good Dog Spot! LLC and any of its employees, agents or volunteers, for any issues or conditions arising from my dog s attendance and participation in daycare. 4. By enrolling my dog in the Daycare program, I am agreeing to follow all policies and guidelines and I certify that I find all the qualifications and practices, including administrative practices and practices regarding the care and precautions for safety, to be reasonable and sufficient. 5. I understand that in enrolling my dog in the Daycare program, the Good Dog Spot! has relied on my representation that my dog is in good health and has never harmed or shown aggression to or made threatening behavior toward any person or dog. 6. I understand and agree that if any problems requiring medical attention should occur with my dog, The Good Dog Spot! will use the veterinarian of their choice and, in the case of not being able to reach me, will make medical decisions on my behalf, excluding euthanasia, and that I will be responsible for all veterinary costs incurred. 7. I understand and agree that any behavior problem or medical emergency will be treated as deemed best by the staff, in their sole discretion, and I will assume full financial responsibility for any and all expenses involved.
