This application is our introduction to you and your environment. Please understand that we form our initial impressions based on the information you give us. If your answers are vague, this will reduce the chances of having your application considered. Please answer the questions in as much detail as possible, even if you are not sure of the answer. Most of the time there are no right or wrong answers, as each dog comes to us with its own specific needs. Checking other or answering as necessary does not provide us with sufficient information to evaluate your application. If none of the options given in our questions seem to fit your circumstances, intentions or desires, please explain in as much detail as possible what you mean, or would like to have happen. This application is very important to us and we take it seriously. The choices we make affect the health and wellbeing of the dog in question for the rest of its life. It is our commitment to make sure we are placing each dog in the most appropriate of all possible homes. Our all-volunteer adoption committee evaluates each application with this intention. If your application is incomplete, we do not have the resources to follow up with you by phone to understand your intentions. If I am not able to keep the dog, or the dog does not work out in my home for any reason, I will return the dog to New Homes 4 Animals. It cannot be given away to anyone or any entity. I CAN make the required contribution (which may differ depending on the animal in question), which helps rescue, provide medical care, spay and neuter, board and place other abandoned dogs and cats. I understand that any contribution is freely given, not a purchase price for a dog. I have read the above information carefully and have filled out this application honestly. DISCLAIMER: There is NO guarantee made, express or implied, that any person requesting to adopt a dog through NH4A Rescue, or posted on NH4A's Website, will automatically be approved. All adoptions are subject to acceptance based on a review process that requires collecting information from references and visiting the applicant's home. Decisions on placing dogs in adoptive and foster homes are based solely on the judgment of NH4A Rescue s volunteers and coordinators, and adoption applications may be denied for various reasons. Also, if you are applying to adopt a specific dog, the volunteers may find that your home would be suitable for a dog but not necessarily for that particular dog that you are applying for, due to its needs and background. NH4A Rescue reserves the right to refuse to adopt to anyone without disclosing the reasons, and to make all decisions regarding placement or final disposition of any rescued dog in its care whether in, or being placed in, a foster or adoptive home or other facility. Any animal (cat or dog) adopted out by New Homes 4 Animals always remains a part of the NH4A family and will be accepted back, no questions asked, at any time during its lifetime. We are juggling scores of applications at once and often have multiple applications on the same dogs. As a result, we may not have time to respond to your application unless we feel it is a good fit for a particular dog.
