3 14:15 15:15 Therapeutic Drug Monitoring (TDM) in Practice 15:15 15:45 Afternoon Tea 15:45 16:45 Small group discussion: TDM case studies At the conclusion of this session, participants should be able to: List antimicrobials that require TDM Understand the concepts of exposure-effect relationships and the situations where TDM will be useful Identify the practical issues surrounding TDM of antimicrobials Describe the role of the pharmacist in the TDM service Gain knowledge various of the software packages available for antimicrobial TDM participants should be able to: Discuss the pharmacokinetic and pharmacodynamic parameters for monitoring vancomycin Compare the various vancomycin dosing guidelines Describe when a loading dose of vancomycin would be indicated Compare and contrast intermittent bolus and continuous infusion of vancomycin Day 2: Sunday 1 st October :30 09:30 Introduction to candidiasis and aspergillosis At the conclusion of this session, participants should be able to: Describe the organisms that are responsible for systemic candidiasis and aspergillosis Appreciate the challenges and approaches to diagnosing systemic fungal infections Describe the patient populations who are at risk of contracting invasive candidiasis and aspergillosis Describe the use of antifungal agents to prevent or manage candidiasis and aspergillosis and the approaches taken when first line therapy fails Understand the management of Pneumocystis jiroveci (carinii)disease in immunosuppressed patients The Society of Hospital Pharmacists of Australia 3

4 09:30 10:30 Small group discussion: Candidiasis and Aspergillosis participants should be able: Discuss appropriate empiric approaches to the treatment of invasive candida and aspergillus infections Discuss appropriate dosing strategies for triazole antifungals including the use of therapeutic drug monitoring Identify and manage clinically important drugdrug interactions with antifungal agents 10:30 11:00 Morning Tea Day 2: Sunday 1 st October :00 12:00 Introduction to Gram positive infections: MRSA, VRE and Streptococcus Infection At conclusion of this session, participants should be able: To know the problematic gram-positive organisms in hospital and community, and the infections they cause To know the antibiotics used in the clinical management of gram-positive infections, in particular for MRSA, VRE and Streptococcus infections, except pneumonia To describe the significant changes to empirical treatment guidelines for MRSA, VRE and Streptococcus infections except Gram positive associated CAP To know the strategies to control emerging antibiotic resistance in gram-positive infections (e.g. application of antimicrobial pharmacodynamics, antimicrobial stewardship, infection control, vaccine development etc.) To have a basic understanding of infection control measures within the hospital 12:00 13:00 Small group discussion: Gram Positive Infections participants should be able to: Discuss the management, antibiotic treatment and monitoring required for infective endocarditis Describe some of the complications of endocarditis Discuss the differences between hospital associated and community associated MRSA infections Describe the different antibiotic treatment regimens for MRSA The Society of Hospital Pharmacists of Australia 4

5 Discuss the role of skin Staphylococcal decolonization and the appropriate treatment regimens 13:00 14:00 Lunch Day 2: Sunday 1 st October :00 15:00 Introduction to Gram negative infections: Pseudomonas, Acinetobacter, Enterobacteriaceae (including ESBLs) TBC 15:00 15:45 Small group discussion: Gram negative infections 15:45 16:00 Evaluation and conclusion At the conclusion of this session, the participants should be able to: List problematic Gram-negative pathogens in hospital and in the community and the infections they cause Identify antibiotics used in practice for the management of Gram negative infections, in particular Pseudomonas spp, Acinetobacter spp, and Enterobacteriaceae (including ESBLs) Describe strategies to control emerging antibiotic resistance in Gram negative infections (e.g. application of antimicrobial pharmacodynamics, antimicrobial stewardship, infection control) including the use of older agents participants should be able describe how to: Discuss the epidemiology, microbiology, risk factors and treatment of urinary tract infections Discuss empiric antimicrobial treatment for urosepsis Discuss the use of microscopy and culture results in infection and translate this to directed treatment Discuss the use of biomarkers in infection Identify considerations for treatment of Gramnegative infections in returned travellers from overseas The Society of Hospital Pharmacists of Australia 5
