2 116 Wound healing 1. Not infected wounds The most simple but nevertheless very effective indication for local laser therapy is the encouragement of wound healing after trauma or operations, in particular when a rapid resilience of the wound closure should be achieved or the wound is located in an area which is difficult to immobilise, such as joints. Infected wounds, that require long term drainage and surgical intervention, heal primarily and suture dehiscence is avoided. Large skin losses heal quickly without complications. Wounds on distal parts of the limb in horses require special treatment, because there is no wound contraction. Thus skin repair happens only by epithelisation, which is a slow process. Therefore horses often develop hyper granulation tissue. 2. Wound infection Laser treatment is highly recommended for infected wounds, fistulas, disturbed wound demarcation and deep wounds. Especially in horses we see the development of hyper granulation tissue in wounds at the limbs. Case report Picture 1 shows a neglected wound with a swelling nearly big like a handball over the fetlock of a foal. The foal had been outside in the pasture night and day. After the wound in the foal was discovered by the owner it got bandages and antibiotics for several days before it came in that state for laser acupuncture. The following therapy included daily LLLT with 6 x 90Watt impulse laser shower (Bio Medical Systems, Germany) for 15 minutes. Additionally LA of SP 2 (wound healing), TH 5, SP 4 (immune stimulation) was treated. No other treatment as antibiotics etc. has been applied. Picture 2 shows the situation 3 weeks after begin of LA: The wound is contracted with good epithelisation and sound granulation tissue. The swelling is reduced half. In picture 3 we see the wound 3 months after begin of LA: The wound is almost closed. In picture 4 healing was finished after 4 month of treatment. All

3 117 over that time the foal was treated nearly every day, most of the time by its owner. I saw the foal every 2 weeks. Picture 1 Picture 2 Picture 3 Picture 4 In other cases we see very often, that the wound only seams to heal on the skin surface and closed but doesn t heal properly inside. Wound demarcation stoped inside the wound and there performed small granulomas under or in

4 118 the scar. These granulomas are the typical disturbing foci or perturbative fields (see chapter 3: Disturbing focus). In cases of badly healed, old scars, laser treatment provoked exacerbation (dehiscence) of the scarred tissue and exudation of necrotic tissue (that had been stored inside the granuloma). So the healing of while first wound closure not demarcated tissue starts again and a final healing will be achieved. The following acupuncture points should be stimulated: LIV 3, SP 2 and to improve demarcation TH 5 and KID 3. In cases of wound infection we add SP 4. What can happen from these granulomas and the way to treat this, I will describe in a case report. Case report A femur fracture in a dog was fixed by a metal plate. After healing of the fracture, the limb of this dog was completely paralysed. There was not any reflex and not any sensivity in the whole leg. Hoping to help the dog, the plate was removed by a second surgery, but the limb didn t improve within the next year. Now the dog came for acupuncture treatment. There was still not any reflex and not any sensivity. The musculature completely disappeared in that leg. It was only skin and bone. The operation wound looked nasty but was closed completely. Beneath and beside the scare one could only feel the femur bone. In the scar I found a strong RAC reaction to laser frequency A. After two treatments with 2 days interim as described above the wound that had been closed for one year opened and stated demarcation (see picture at the tip of the big arrow, the small arrows show the size of the nasty scar). Fistulation in a 1 year old closed wound starts after two laser acupuncture treatments (tip of the big arrow), the small arrows show the size of the nasty scar.

5 119 While the next two days it became more and more swollen and showed fluctuation. A big abscess developed and after further two days. 150 ml pus came out when the abscess opened. Now I started to treat the neurological problem, treating the Shu points from Kidney to Large Intestine, that all showed strong RAC reaction with frequency C and the Governor Vessel between the spines with frequency E, according to the spinal cord in the same area. Also a strong reaction was shown on frequency E medial along the femur bone. I believe it was a part of the Ischias nerve. Additionally the point KID 4, the influential point of nerve tissue was treated with Fr. E. This treatment was carried out for one further month daily by the owner itself with a rented impulse laser 90 Watt. When the patient came for the next visit after this time, the dog was able to stand on the leg and to move a little bit. All reflexes could be generated and the skin sensivity was back again. The treatment was stopped. At the next visit in my praxis 2 month later the dog walked and run perfectly and the muscles had grown up again. The dog was completely restored. 3. Abscesses and phlegmonia Laser treatment is also exceptionally useful in local inflammations. In very many cases phlegmonic processes, e.g. after the infection of wounds, in mastitis and even in acute to sub-acute thrombophlebitis can be cured when previous treatment with antibiotics have proved ineffective and much easier in fresh infections before antibiotics have been applied. Laser irradiation also has an outstanding effect on the maturation and demarcation of abscesses (see chapter 1: Horse with abscess in the fore chest). This also applies, for example, to hoof ulcers or infected inflammations of the hoof dermis which do not mature and diffusely spread into the rest of the hoof dermis and which are normally very difficult to manage.

Purulent, acute and chronic sinusitis in dogs and even in horses can be effectively treated with laser acupuncture, whereby in horses the affected tooth usually does not have to be extracted and the maxillary sinus does not have to be trepanned (see chapter 1 Horse with unapparent frontal sinusitis). Lastly, I would like to cite the highly effective option of treating infected joint and tendon sheath inflammations. At commensurate cost, in most cases excellent treatment success can be expected here (see chapter 6 case study No. 1 and 2). Indicated acupuncture points are : TH 5, SP 4, GB 41, KID 3 4. Infections of the navel and fistulas of the urachus Navel infections often cause navel hernia in dogs and foals. Especially in foals we also see infections starting from the navel and leading to severe general joint infections. In foals we also see sometimes that the urachus doesn t close after the umbilicus cord is ruptured and urine ins dropping out of the umbilicus. With laser acupuncture we can do the best prophylaxis I can imagine to all these problems. We treat the navel with frequency A and B for 30 sec each and 3H 5 and SP 4 (fr. 5). In cases when infection already took place we must treat the navel much longer (3-5 minutes with Frequency A
