2 VETERINARY CHECKLIST A printout or letter from your vet is required showing your pet s medical records for the past year. This document must show dates of the following. Dogs Bordetella - Kennel Cough (Within the past 12 months) DHPP Vaccination Distemper, Hepatitis, Parvo, Parainfluenza (Within Past 12 months) (Both Bordetella and DHPP are available at Hector s Hay & Feed for purchase.) Rabies Cats Initials Cat vaccinations are not required, but Upper Respiratory is HIGHLY recommended Feeding Instructions We will supply Victor pet food at no additional cost or you may bring your own. Morning Feeding - Cups Evening Feeding - Cups Additional Information Please note that if your pet is found to have fleas, ticks or worms upon arrival they will be given a flea bath and/or wormer at the owner s expense. 3 Kennel Charges The nightly rate for one dog is $ Additional dog, sharing the same kennel, is $ Cat Boarding rates are $14.00 per night. Additional cat, sharing the same kennel, is $8.00. Please note there is an additional fee of $10.00 for Late Check In or Check Out Doggy Daycare A full day rate is $16.00 and $10.00 for half days (under 4 hours). Doggy Daycare hours are Monday thru Friday 9AM to 5:30PM (There is an additional $10.00 fee for early drop-offs or late pick-ups) Please schedule ahead of time if you want to drop off your dog early. Additional Amenities We offer a range of additional services to your pet when they stay, please check any of the following you would like to receive. Dog Bath Staring at $ (10% off with boarding) Cat Grooming Staring at $ (10% off with boarding) Full Groom Starting at $ (10% off with boarding) (Please see attached grooming price sheet for additional pricing information.) Nail trimming $10.00 Anal Gland Expression $10.00 Daily Walks $3.00 per walk One on one training with our certified trainer. Price Includes one daily walk on day of training. 1 Week Board & Train - $ plus Boarding Fees 2 Weeks - $ Plus Fees - 3 Weeks - $ Plus Fees This is a Pet Boarding Contract (hereinafter this Agreement ) between Hector s Hay & Feed LLC (hereinafter called Kennel ) and the pet owner whose signature appears below (hereinafter called Owner ). 1. Kennel agrees to exercise due and reasonable care to keep its premise sanitary and properly enclosed.
4 2. Owner understands and accepts that there is a known presence of potentially harmful pests in the area in which the Kennel cannot be held responsible for exposure. (This includes, but is not limited to, rattlesnakes, ticks, scorpions, centipedes, etc.) 3. Owner agrees to pay the rate for boarding in effect of the date Owner s pet is accepted for check in by Kennel. Owner further agrees to pay all costs for the pet during the time said pet is in the care of the Kennel. 4. Owner understands and explicitly agrees to be solely responsible for any and all acts of behavior of his or her pet occurring during the term and time of this Agreement. Owner specifically waives and agrees to release Kennel from any liability with respect to their pet s character or nature whatsoever, arising out of or from boarding this pet, or any and all damages which may accrue from any other cause whatsoever including loss by fire, theft, running away, death, injury to persons, animals or property during the term of this Agreement, whether this pet be on the premises or not. 5. Owner understands that in no event shall the responsibility and/or liability of the Kennel exceed the sum of Two Hundred Dollars ($200.00) and Owner agrees to limit the responsibility to $ for any and all damages sustained or suffered by reason of the boarding of this pet. Owner further agrees not to claim any damaged against said Kennel of any nature, either by way of contract, equity, negligence or otherwise, in excess of said sum. 6. Guests are charged for their night of arrival. Check-out time is between 9AM and 12PM. Guests who depart after 12 PM will be charged an additional $10.00 late pickup fee. 7. During peak times (i.e. holiday weekends or breaks) there will be a non-refundable deposit of 50% of the estimated boarding. The remaining fees are due at check-out time. 8. Owner does hereby entrust Kennel to care for said pet and authorizes Kennel, in its sole discretion, to perform whatever is needed for the health and well-being of the Owner s pet. Owner further authorizes Kennel to take their pet to a veterinarian of the Kennel s choice and agrees to pay for and all expenses relating to the pet s care, transportation and board. Owner agrees to hold Kennel harmless for any illness, accident or injury and agrees to be solely responsible for any and all damages that pet may cause to Kennel and Kennel s staff, other animals or guests. Kennel will make every effort to contact owner or primary contract, but will perform as above if unable to do so. 9. In the event of undesirable pet behavior, which include, but are not limited to: signs of aggression, separation anxiety, excessive barking, or destructive behaviors, Owner agrees that Kennel may crate the Pet. If this action does not solve the problem, Kennel will notify the Owner or Primary Contact by telephone or and Owner agrees to promptly remove pet from Kennel s premises. Undesirable Behaviors shall be defined solely by Kennel. Signature Date
