2 Bitch Owner s Cell Phone: Bitch Owner s Home Phone: Bitch Owner s Work Phone: This agreement is between Helena B. Lamont, owner of the above stated stud dog, and the above Bitch Owner. The stud fee is $1, U.S. Dollars. A fee of $ U.S. Dollars (non refundable breeding fee) shall be due at the time of breeding leaving a balance of $1, which will be due when the puppies turn eight weeks old and/or before the stud dog owner agrees to sign the Litter registration (and no later than the puppies are six months old). The stud dog owner reserves the right to request a puppy back in lieu of the balance payment for stud service. In the event the stud dog owner opts for a puppy back the stud fee will be waived, the deposit returned, and the Stud dog owner will pay the bitch owner an additional $ The Stud dog owner will take second pick of the litter if the bitch owner is keeping the first pick. If the bitch owner is not keeping a puppy, then the stud dog owner will have the option to take the first pick puppy. If the stud dog owner decidedly takes a puppy instead of a stud fee said puppy is given free and clear to the Stud dog owner on an Open registration and in sole name of the Stud dog owner.

3 The Stud dog owner retains the right to refuse signing the Litter registration until the bitch owner has paid the balance in full for the stud service whether it be with a cash payment in U.S. Dollars or upon receipt of a puppy back, and is reimbursed for any veterinary cost or other services which may have been incurred at the time of the breeding. Ms Lamont will submit any and all outstanding bills to the bitch owner for verification and reimbursement purposes. A litter is defined as three or more puppies. If two or less puppies are born, the stud fee will be $ per puppy (over and beyond the non refundable breeding fee) and there will be no further obligation on part of the stud dog owner or bitch owner. If no live puppies result from the breeding, no additional payment will be due to the stud dog owner and another breeding to either the same bitch will be offered or another one owned by the bitch owner without a breeding fee. The bitch owner assures that said bitch is in good health at the time of breeding, is current on her vaccines, and has had a bordatella vaccine in the last six months if she is to reside at Ms Lamont s home. The bitch owner also agrees to provide the stud dog owner with the following: Copy of the bitch s registration Three generation pedigree of said bitch Test breeding in K9 Data Base OFA/ OVC/Penn Hip certifications for Hips and Elbows

4 A current CERF/OFA Eye certification performed in the past year OFA Cardiac certification or report from a board certified cardiologist Proof of brucellosis test done within 10 days of breeding Copy of immunization records Photograph of said bitch The bitch owner is responsible for any and all transportation of the bitch to and from the stud dog owner s residence. If the bitch is shipped by air to and from the Atlanta airport, the bitch owner agrees to pay all shipping costs in advance. Under no circumstances is the stud dog owner responsible for any shipping costs of said bitch. Ten days of boarding for said bitch is included in the stud fee; if the bitch remains at the stud dog owner s residence longer than ten days, a fee of $40.00 per day will be incurred. Ms. Lamont will do everything in her power to keep said bitch happy and safe. Said bitch will be kept in a temperature controlled environment at all times, have access to fresh water at all times, be fed twice a day or as instructed, given any medications prescribed by the owner s request or approval, and be let out in a securely fenced area throughout the day to exercise and go potty. The bitch owner will provide the stud dog owner with the bitch s food, supplements, feeding instructions, treats, and toys for the bitch. She will also provide the stud dog owner

5 with any medications or special instructions relating to the bitch. If the stud dog owner has to provide any of the above it is understood and agreed upon that she will give the bitch owner any and all receipts which the bitch owner will reimburse the stud dog owner for before releasing said bitch back to it s owner. It is understood and agreed upon that the stud dog owner is not responsible if said bitch becomes sick, has an accident, or expires while at the stud dog owner s residence. Should something happen to the bitch during the time she is at the stud dog owner s home, Ms Lamont will notify the bitch owner immediately. In the event she cannot get hold of the bitch owner and Ms Lamont feels it is necessary to seek immediate medical attention for said bitch, the bitch owner authorizes her to do so. Any veterinary costs incurred by the bitch will be the responsibility of the bitch owner. It is agreed upon that under no circumstances is the stud dog owner responsible legally or financially for any illness, injury, or death, which may occur during or after this breeding. It is agreed upon that any uncontrollable, aggressive or destructive behavior on part of the bitch will be cause to return said bitch to her owner and make this contract null and void with no monetary reimbursements on part of the stud dog owner. Should said bitch cause injury to one of Ms Lamont s dogs or cause damage to her property, the bitch owner will be responsible for any and all veterinary costs and any repair costs.

6 It is understood and agreed upon by the bitch owner that all expenses related to collections and shipping of semen, and inseminating the bitch, which may include but are not limited to: costs for blood work, smears, progesterone tests, AI s, surgical implants, trans cervical implants, medications, and any boarding expenses are the sole responsibility of the bitch owner which she will pay directly to the clinic for. In the case of natural breedings, said bitch will be bred once a day or once every other day as soon as she will allow until she will no longer accept the stud dog unless otherwise instructed by the bitch owner. All breedings will be monitored and supervised by Ms Lamont to ensure the safety of both dogs. The bitch owner agrees to provide the stud dog owner with the names, addresses and phone numbers of all clients that purchase puppies produced from said breeding. Bitch owner also agrees to provide the registered names and AKC/CKC registration numbers of each puppy to the stud dog owner. Bitch owner agrees to sell all pet puppies on a Limited Registration with a spay/neuter/non breeding requirement in her sales contracts. She also agrees to sell any show puppies on a co-ownership with either herself or the stud dog owner being on said puppy s registration unless said puppy is being sold to a well known, proven show home. It also must be stated in her contract that any show puppies must pass their clearances for hips, elbows, eyes, and hearts after turning two years of age before being bred.

7 Bitch owner agrees NOT to sell any puppies produced by said breeding to pet shops, brokers, agents, or any commercial establishments. Bitch owner also agrees NOT to sell any puppies outside the United States or Canada. Upon agreeing to the above conditions by both parties, the signing and dating of this contract will make it mutually acceptable and official. Stud dog owner signature- Helena B.
