1 OFFICE USE ONLY DATE SUBMITTED: APPROVED: YES NO GROUP SERVICE APPLICATION Please complete this application and return it to our Volunteer & Community Engagement Manager so that we may schedule your group service day. Applications must be submitted at least 30 days in advance of requested date(s). Thank you for your interest in volunteering with the Lawrence Humane Society! We look forward to partnering with your organization to improve the lives of homeless companion animals in our care. In order to create the most fulfilling experience for your group, and match you with an appropriate activity, we ask that you fill out the following application. Once submitted, our Volunteer & Community Engagement Manager will respond within 7 business days. If you have any questions about this application, available activities, or the group requirements, please volunteer@lawrencehumane.org. ORGANIZATION: ADDRESS: Contact Person: Phone: Date Requested: Second Choice: Group Size: Preferred Time Frame (Please Check One) o 9:30a 12:30p o 11:00a 2:00p o 2:00p 5:00p o Other: 1 P a g e
2 GROUP SERVICE OPTIONS (Please Select One) The following activities are available on location at the Lawrence Humane Society. Your service time will include a 30 minute tour of the facilities, with the balance of time used on volunteer activities. o Kitty Korner Reading Minimum Age: 7 Maximum Group Size: 10 Need to boost the confidence of your young readers? Sign them up for Kitty Korner Reading groups! Children aged 7 10 can bring their favorite book and read to the cats in one of our three adoption areas. Maximum of 8 students. A minimum of two teachers will also need to accompany the children. o Beautification & Enrichment Minimum Age: 10 Maximum Group Size: Life at the shelter can be stressful for the animals, so we seek to make their stay as comfortable and stimulating as possible while we are caring for them. Clean kennels and well-kept grounds are vital to ensuring the health and happiness of our animals, and making our shelter an attractive option for potential adopters. With this project, volunteers will assist with essential grounds keeping, shelter cleaning tasks and/or creating enrichment toys. Tasks will include, but are not limited to: laundering animal linens, refilling water bowls, weeding, cleaning kennels, washing & sanitizing toys, et al. Due to the nature of the work, we may not be able to inform you of the exact tasks before you arrive at the shelter. During winter months, maximum group size is restricted to 10. OFF-CAMPUS SERVICE OPTIONS The following activities are available options for groups. These tasks can be completed off site, and delivered to the shelter. Instructions can be found on the last page of this application. If you have another idea, please share. o Make Cat Blankets Cats love a nice, soft bed to lie on, and an attractive kennel draws potential adopters. You can crochet or knit 15 x15 blankets, or use felt to make a cozy corner for our cats. 2 P a g e
3 o Host a Donation Drive Organize a drive to collect much needed items for the shelter! Items we always need include: Blankets Bleach (Unscented) Dog & Cat Treats Peanut Butter Catnip Kongs Tennis Balls Pet Toys Hand Sanitizer Office Supplies Scrubbing Sponges Dawn Dish Soap (Blue) Pet Carriers Et al o Make Enrichment Toys Life at the shelter can become rather stressful for the animals. Having toys breaks up the boredom and stimulates their minds. Whether for dogs or cats, toys keep our animals healthy and happy while they wait for their forever homes. o Organize a Fundraiser Have a bake sale or car wash to benefit the animals. Hold a Pennies for Puppies drive, and decorate canisters to collect coins to support the shelter. There are several ways to raise funds for the shelter. Due to the volume of requests, the Lawrence Humane Society is unable to take on any organizing, administrative, and/or staffing for the event. A Third Party Fundraising Agreement must be signed in advance. o Other Please explain your idea below: 3 P a g e
4 GROUP SERVICE AGREEMENT The Lawrence Humane Society policies regarding group service events are as follows: 1. Applications must be submitted at least four (4) weeks in advance of the requested service date. 2. To allow our staff enough time to properly prepare for your group, a final headcount is required three (3) business days before your assignment. 3. To provide the most meaningful experience for every group interested in serving, we limit approved requests to two (2) group service assignments per month. 4. Applications are approved on a first come, first served basis, with group assignments approved in the order in which they are received. 5. If we are unable to accommodate a group due to space, they will be placed on a waitlist. If a scheduled group cancels, then the waitlisted groups will be contacted in the order they applied. 6. All volunteers must wear long pants and closed-toe shoes, with long hair secured away from the face. 7. Any volunteers under the age of sixteen (16) are considered Junior Volunteers and must be accompanied by a parent or guardian. 8. Groups must maintain a ratio of one (1) adult for every three (3) junior volunteers. 9. Liability waivers will be ed to the group contact after a date is scheduled, and must be returned by the date of service, before any volunteer work begins. 10. To maintain the safety of our guests and animals, groups will not work directly with the animals. However, if time and staffing permits, an animal may be brought out to meet the group at the end of their assignment. 11. Cancellations received with less than seventy-two (72) hours notice may be result in a group being automatically waitlisted when they next submit a request. I have read the above policies and agree, in the event of this request being approved, that my group will abide by them. Group Contact Signature: Date: 4 P a g e
