I this article, I made a list of the top 10 pieces of advice that I would have give myself a year ago: Use a step-by-step approach. You do t have to address every abormality from the physical exam durig the visit. Most cliets do ot wat to do everythig at oce, either. You will gai their trust if you prioritize ad outlie your pla, usig if, the statemets. Sped your emotioal eergy o the cliets who appreciate it ad get the job doe efficietly for those who do t. Do t be discouraged by the miority of cliets who may just see you as a service. There are plety more who are proud to call you their veteriaria for life! You do t have to have all l the aswers, or do you have to kow them right away. Make your assessmet ad pla ad do t uderestimate the value of cosultig with a specialist whether that be a laboratory represetative, your local emergecy cliicia, or specialists from your alma mater. Develop a professioal volume xii umber 3 fall 2017 iche. What is goig to differetiate you as a veteriaria? You do t have to determie this right away but as you gai experiece, pay attetio to the thigs that excite you. Learig is a life-log process. Lear from your cases that day ad prepare for the oes tomorrow. Ad the leave your work at work. You ve got the rest of your career to accomplish more firsts. Set self-care boudaries. Fresh air ad walkig your dog Romeo over luch improves your quality of life ad makes you more efficiet whe you retur to work. Have the disciplie to make sure you give yourself that. Cattle ifected with BLV are usually asymptomatic. However, approximately percet of aimals become persistetly lymphocytic, ad 5 10 percet develop tumors such as lymphoma ad leukemia. I additio, ew evidece suggests that sub-cliical ifectios may actually be impairig immue fuctio, which ca icrease risk for other ifectious diseases. IMPACT TO INDUSTRY BLV also ca have a cosiderable ecoomic impact to dairy operatios. Cattle ifected with BLV have a 23 percet greater risk of dyig or beig culled whe compared to BLV egative cows, 1 have decreased milk productio, 2 ad reduced reproductive efficiecy. 3 Furthermore, BLV iduced lymphomas are the most commo reaso cattle are codemed at slaughter. 4 Accordig to a 1997 study from the U.S. Departmet of Agriculture Natioal Aimal Health Moitorig System, about 38 percet of all beef cow-calf operatios have at least oe positive aimal ad overall 10.3 percet of aimals tested positive for BLV. 5 I 2007, NAHMS studied prevalece o US dairies ad foud that 89 percet of dairy operatios had at least oe positive aimal, ad overall about 50 percet of all dairy cattle are ifected. 7 I Michiga, 87 percet of dairy herds had at least oe BLV positive aimal, ad herds with at least oe BLV positive aimal raged from percet of their aimals testig positive for BLV. 8 Outside of the US, Irelad, the Uited Kigdom, Spai ad approximately 17 other coutries i Europe have eradicated BLV. 9 These coutries used a test ad cull or segregate method to eradicate the virus. Sice levels of BLV are so high i the US, this method will ot be the most ecoomical to producers. Research is curretly uderway at the Michiga State Uiversity College of Veteriary Medicie to fid cost-effective methods to reduce BLV prevalece i herds. TRANSMISSION Successful prevetio ad cotrol of BLV ifectio requires uderstadig how the virus is trasmitted. Trasmissio occurs by trasfer of B lymphocytes i mediums such as blood, milk ad asal secretios. 10 Iatrogeic procedures that ivolve the trasfer of ifected blood accout for the majority of ew cases. These procedures iclude rectal palpatio, ijectios, tattooig, ad dehorig. REDUCING BLV IN HERDS Methods to reduce trasfer of BLV i a herd iclude the followig: Usig oe eedle per cow. Disifectig or chagig obstetric sleeves betwee cows. Maagig flies ad other bitig isects, sice bitig isects ca act as a mechaical vector for the virus. Feedig colostrum from BLV egative cows or freezig colostrum prior to feedig it to calves. Pasteurizig milk fed to calves to destroy the virus as well as other harmful pathoges that ca be trasferred i raw milk. Workig together with producers to improve maagemet practices ca help reduce the spread of BLV i herds. However, BLV is a lifelog ifectio, ad eradicatig BLV from a herd will evetually iclude the cullig of aimals to reduce the umber of positive aimals i the herd. STATE VETERINARIAN RESPONSE BLV is a reportable disease i Michiga. The umber of positive aimals is reported to the Michiga Departmet of Agriculture ad Rural Developmet s Aimal Idustry Divisio (MDARD AID) i order to follow the spread of the virus. Curretly MDARD AID takes o actio o reported cases, ad o ivestigatio occurs whe a positive herd or aimal is idetified. BLV is moitored to allow a uderstadig of curret prevalece i the state. As the idustry moves forward, BLV may affect import or exports with coutries that have bee able to eradicate the virus. As the

3 prevalece of BLV cotiues to climb, it will become more difficult to eradicate from a herd with miimal ecoomic impact. It is importat to modify maagemet practices by workig with producers to reduce spread of BLV, ad therefore reduce the impact of the virus o herds ad their owers. I late 2013, the State Veteriaria ad Divisio Director of the Aimal Idustry Divisio of the State of Michiga issued a otice to Michiga veteriarias statig, I recet moths, it has bee brought to my attetio that persos i our state may be violatig State Law by practicig veteriary medicie without a licese. The State Veteriaria urged the professioal commuity to iform potetial offeders of the state licesig laws ad file complaits about persos practicig veteriary medicie without a licese. The Michiga Veteriary Medical Associatio takes such matters very seriously ad regularly otifies idividuals it becomes aware of who may be practicig veteriary medicie without a licese, whether it is because the idividual s licese has lapsed but still practicig veteriary medicie, or because the idividual is performig specific procedures o a aimal, such as spial maipulatio, without proper supervisio by a licesed veteriaria. The practice of veteriary medicie is defied i the Michiga Public Health Code to mea: Prescribig or admiisterig a drug, medicie, treatmet, or method of procedure; performig a operatio or maipulatio; applyig a apparatus or appliace; or givig a istructio or demostratio desiged to alter a aimal from its ormal coditio. Curig, amelioratig, correctig, reducig, or modifyig a disease, deformity, defect, woud, or ijury i or to a aimal. Diagosig or progosig, or both, a disease, deformity, or defect i a aimal by a test, procedure, maipulatio, techique, autopsy, biopsy, or other examiatio. 1 Practicig or holdig yourself out as practicig veteriary medicie without a licese is a feloy puishable by imprisomet for up to four years, a fie of up to $5,000, or both. 2 Aidig someoe else i practicig veteriary medicie without a licese is a misdemeaor puishable by imprisomet for up to 90 days, a fie of up to $100, or both. 3 Allowig ulicesed idividuals to practice veteriary medicie is a threat to public health ad safety. The state licesig laws were established to protect the public s geeral health, safety, ad welfare. The Michiga Board of Veteriary Medicie (Board) ad the Departmet of Licesig ad Regulatory Affairs (LARA) are statutorily charged with eforcig the licesig laws. They have the statutory authority to seek ijuctive relief through the Attorey Geeral s office or a prosecutig attorey of the couty i which a violatio occurs. 4 Similarly, the couty prosecutig attorey ad the Michiga Attorey Geeral have specific statutory authority to eforce the licesig laws. 5 This authority is well recogized ad publicized o the LARA website for the Board: The Board also has the obligatio to take discipliary actio agaist licesees who have adversely affected the public s health, safety, ad welfare. Likewise, the judiciary plays a importat role i upholdig our state licesig laws. I oe reported case, the Michiga Court of Appeals reversed the Board of Chiropractic Discipliary Subcommittee s dismissal of a cease ad desist order issued by the Bureau of Professioal Licesig. The cease ad desist order was issued agaist a licesed chiropractor to stop performig chiropractic maipulatio o aimals. He did ot have a veteriary licese ad was providig chiropractic treatmet to horses. At the coclusio of a admiistrative hearig, the hearig officer, who is resposible for determiig the facts ad issuig a proposal for decisio to the discipliary subcommittee, foud that the chiropractor was performig spial adjustmets o horses. The hearig officer further cocluded that the care ad treatmet of aimals was outside the scope of chiropractic ad specifically icluded i the scope of veteriary medicie. Therefore, the practice of equie chiropractic by a licesed chiropractor, who was ot a licesed veteriaria, was illegal. The discipliary subcommittee accepted the hearig referee s fidigs of fact but disagreed with the legal coclusio. The discipliary subcommittee ruled that there was o law that limited the practice of chiropractic to humas. Cosequetly, because the chiropractor was merely performig spial maipulatio, which is withi the scope of chiropractic, there was o violatio of the licesig laws. That rulig was appealed, ad the Court of Appeals disagreed ad held that the scope of chiropractic does ot iclude the treatmet of aimals. The court further held that the actio of performig spial adjustmets o horses is cotemplated by the statutes regulatig veteriary medicie... the Legislature did ot ited for chiropractors to practice chiropractic o aimals. Further, the cease ad desist order was properly etered to ejoi [the chiropractor] from performig chiropractic maipulatio o horses or other aimals without havig a licese to practice veteriary medicie. 6 Had the chiropractor bee uder the supervisio of a veteriaria, he may have bee able to lawfully perform the chiropractic maipulatios. The availability of a licesed health professioal o a regularly scheduled basis to review the practice of the supervised idividual, to provide cosultatio to the supervised idividual, to review records, ad to further educate the supervised idividual i the performace of the idividual s fuctios. The provisio by the licesed supervisig health professioal of predetermied procedures ad drug protocol. 7 Licesed veteriarias i Michiga are permitted to delegate the performace of tasks that costitute the practice of veteriary medicie provided the veteriaria has first examied the patiet o which the delegated procedures are to be performed ad determied the eed for such veteriary services. 8 The admiistrative rules goverig licesed veteriarias also requires the delegatig veteriaria to observe ad moitor the performace of the delegated procedures to the extet ecessary to esure that the activities of the delegate are withi the scope of the orders, assigmets, or prescriptios of the veteriaria. 9 However, eve assumig a veteriaria satisfies the legal requiremets for supervisio, it does ot mea that veteriarias ca completely relegate licesig resposibilities to someoe who is ot a licesed veteriaria. For example, the Michiga Departmet of Agriculture ad Rural Developmet issued expected protocols for rabies vacciatio of dogs i public ad private settigs. I additio to recommedig accredited ad Michiga licesed veteriarias to follow the Compedium of Aimal Rabies Prevetio ad Cotrol, 2016 ad maufacturers label for admiistratio of rabies vaccie, the Departmet expects a licesed veteriaria to examie the dog ad admiister the rabies vacciatio. The veteriaria may direct a licesed veteriary techicia to vacciate the dog as log as the veteriaria is close eough to the veteriary techicia ad the dog to be able to observe ad moitor the veteriary techicia s activity. 10 Supervisio of a veteriary techicia icludes that degree of close physical proximity ecessary for the supervisig veteriaria to observe ad moitor the performace of a veteriary techicia. 11 I oe case, a veteriaria had his licese suspeded for oe year ad was ordered to pay a civil pealty for kowigly employig a ulicesed perso to practice veteriary medicie. The licesee employed a ulicesed veteriary school graduate to perform surgeries o aimals. The court foud that the licesee kew the graduate was ot licesed because the graduate failed to cospicuously display ay licese. 12 I Michiga, licesed veteriarias are required to visibly display a curret licese to practice veteriary medicie where it is visible to the public i the licesee s pricipal place of busiess. 13 I aother case, a licesed veteriaria allowed a ulicesed employee to perform euterig surgeries o male shelter cats. The appellate court upheld the Board of Veteriary Medicie s fidig that the procedures costitute the practice of veteriary medicie. The court upheld the veteriary board s decisio to sactio the veteriaria. 14 LARA ad the Board are ot the oly overseers of the licesig laws. Licesed veteriarias who have kowledge that aother licesee has violated the licesig laws have a ethical duty to act ad legal duty to report the idividual to LARA. The AVMA Priciples of Veteriary Medical Ethics provide, A veteriaria shall uphold the stadards of professioalism, be hoest i all professioal iteractios, ad report veteriarias who are deficiet i character or competece to the appropriate etities. 10 MAHF Fuds Three Research Grats mvma s oprofit arm, the Michiga Aimal Health Foudatio (MAHF), is a charitable trust fouded i 1969 to give veteriarias ad their cliets a opportuity to fud critical research that beefits the health ad welfare of aimals. Over the years, the foudatio has bee ivolved i a wide variety of research projects for both small ad large aimals. The MAHF Board of Trustees met i Jue ad evaluated several excellet proposals. Trustees agreed to provide grats for three of the research projects: IS BRINE FINE? EFFICACY OF A SATURATED SALT SOLUTION AS AN ALTERNATIVE TO FORMALIN FOR STORAGE OF ANATOMY SPECIMENS Formali is a kow carcioge that has bee a staple for maitaiig specimes for teachig veteriary aatomy for may years. The upleasat smell ad huma health risks are substatial ad take away from the learig eviromet. Studets are icreasigly reluctat to sped time dissectig cadavers ad examiig prosectios (pre-dissected ad labelled specimes made by the istructors to demostrate muscles/erves/vessels that are used over may years) due to the oxious ature of formali, especially as side effects ca iclude difficulty breathig, rashes, burig sesatios i the eyes ad throat, ausea, chest tightess, ad headaches. This ca impact a studet s competecy i aatomy, which is a essetial buildig block for both future coursework ad success i veteriary medicie ad/or veteriary techology. To reduce the formali exposure, the aatomy departmet at MSU CVM has udertake storage of specimes i a saturated salt solutio essetially briig our specimes to preserve them. Aecdotally, this solutio works just as well to preserve specimes as the more carciogeic formali ad at a fractio of the cost. This project would evaluate the ability of various kow specime storage solutios (formali, saturated salt, Wardsafe ad Klotz solutio) to prevet mold growth, maitai specime itegrity, ad reduce the smell of preserved aatomy specimes over the course of three moths. It is hypothesized that iitially preservig the specime i formali followed by brie log-term storage provides less formali exposure ad a better quality teachig aid tha other commo specime storage solutios. Oxidative stress results from a imbalace betwee the productio of reactive oxyge species (ROS) by immue cells durig iflammatio ad the depletio of protective atioxidat defeses. Major sources of atioxidats are derived from microutriets provided i the diets of dairy cows. Vitami D is recogized as a atioxidat microutriet i huma medicie. I recet years, there also is compellig evidece to suggest that itramammary vitami D treatmet ca reduce the severity of mastitis. 12 Compaio Aimal Fud Success Stories Kimber Kimber arrived at Hall Veteriary Cliic with a very paiful ear caused by a ifectio. While the mai focus of the MVMA legislative team has bee o amedig the opioid bills that are curretly movig through the legislative process, it s worth otig three other major issues that passed both chambers of the legislature ad were siged by Goveror Rick Syder. To start, the Goveror siged the $56.5 billio spedig pla for fiscal year While there were a few vetoes i the budget, it did come as a surprise, give the attetio lawmakers have bee givig opioid addictio. Goveror Syder vetoed a $850,000 Geomic Opioid Abuse Demostratio Program that would predict opioid respose ad abuse. Accordig to the Michiga Departmet of Health ad Huma Services (MDHHS), the allocatio could have jeopardized other federal moey. Secodly, the cotetious teacher pesio reform bill (SB 401) that gives school istructors a more eticig 401(k) optio but also makes the pesio optio less attractive, is ow law.
