1 770 Coopertown Road, Delanco, Ni (856) COpy Initial Investigation Report Date: February 1, 2015 Department Case # New World Case # On the above date at 0000 hours Delanco Police were dispatched to 902 Hickory Street for a dog attack. I, Ptl. Joel Mufalli, responded in my marked patrol unit with lights and sirens. In route to the scene, Central Communications gave further information that an individual was currently being attacked. Ptl. Welch and I arrived on the scene within moments of each other and positioned our vehicles across the street from 902 Hickory Street. Upon arrival I observed a man in a white T-shirt and sweatpants standing in the middle of the street out front of 902 -Hickory. The man had his arms in the air and was acting frantic and upset. This man was later identified as Nicholas S. Dalessandro, a resident of 902 Hickory. Dalessandro was excited and made remarks about his dog attacking a female in the residence. Dalessandro appeared uninjured and was pleading for the police to do something. After observing Dalessandro, Ptl. Welch and I approached the residence with our weapons drawn. Ptl. Welch quickly looked through a window and observed a female being attacked by a dog. I went to the front door where an individual was leaning against a chair with a four to five inch open wound on his left leg and blood all over both legs. This individual was later identified as allatik a resident of 902 Hickory. He stated that the dog was attacking his mom. NI. Welch and I then entered the residence, myself as the primary officer and NI. Welch as secondary. Inside I heard a dog barking and a woman screaming for help. We quickly moved through a dining room that looked as if a commotion taken place. We followed the screaming and found a woman later identified as Armida Jacobs on the family room floor. Ms. Jacobs was covered in blood and had wounds on her legs. I observed a Rottweiler biting down on Ms. Jacobs's lower shin and shaking its head violently. At this time I raised my weapon towards the attacking dog. I aimed for a clear shot at the body of the dog. Once a clear shot was obtained I fired multiple times at the dog. The dog then rolled onto its rear and landed on its left side facing away from me. I still observed movement from the dog so I obtained a clear shot and fired my weapon once more into the dog, which finally stopped moving. There was another dog in the room that was not a threat. _ I was notified by Ptl. Welch that there was a third dog in the residence. I reloaded a fresh magazine into my I weapon, holstering the one I had seated in my duty firearm. I observed the loose dog to my rear and quickly blocked him in a room with kitchen chairs. Ptl. Welch and I immediately began first aid on Ms. Jacobs. Sgt. Urtasun from District 70 then entered the room with a medical bag. Sgt. Urtasun and I cut the sleeves and pantlegs of Ms. Jacobs to gain access to the wounds. I observed multiple open puncture wounds and lacerations on the right forearm, wrist and hand. I then placed a sterile gauze pad on the wounds and began to wrap the entire affected area. Once fmished, I assisted in the treatment that Sgt. Urtasun was giving on the legs of Ms. Jacobs. By this time, two Delran BLS units arrived and a crew began treating Ms. Jacobs. During treatment Ms. Jacobs stated that she didn't know why her dog would do that to her. Once treatment was turned over to Delran EMS I asked for further instruction from Sgt. Tilger, who had arrived on scene moments after Ptl. Welch and I. Sgt. Tilger requested that I get Mr. Jacobs's information and notify NI. Welch to get information from Ms. Jacobs. I notified Ptl. Welch of the same, and then took Mr. Jacobs's information inside the E residence. The only information that he gave me was that the attack started sometime around 0000 hours. After receiving his identifying information, EMS 239 transported both Mr. Jacobs and his mother to Cooper Hospital Trauma. Mr. Dalessandro remained at the residence and stated that two dogs started fighting over food in the kitchen, and that one began attacking Ms. Jacobs and her son when the residents tried to break it up. The scene was then secured until Det. Russell arrived on location. At this point Sgt. Tilger released me to Police Headquarters where I was evaluated by Delanco EMS and refused medical treatment. 3 COPY Supplemental Investigative Report Date: 02/01/2015 Department Case # New World Case # On February 1, 2015 at approximately 0000 hours, Sergeant A. Tilger # 1111, Patrolman J. Mufalli, and I, Patrolman H. Welch # 1123 were dispatched to 902 Hickory S eet in reference to a o attack. Upon arrival I observed two frantic white males who were later identified as, and Nicholas Shayne Dalessandroalla, bkppeared injured (lacerations and blood on both legs). As the police approached, both mentioned subjects in a distraught tone informed us that one of their dogs was attacking their mother/aunt. At which time, they pointed to the front bay window. I quickly approached the window to get an assessment of the incident and of the location of the other dogs inside of the said residence. I was aware of multiple dogs residing at the said residence from a previous 911 call. Through the window I observed a dog what appeared to be a Rottweiler breed attacking the leg of a woman. The woman was later identified as, Armida Jacobs , After making a quick assessment, I quickly approached the front door, and I observed Patrolman J. Mufalli entering into the said residence. Patrolman J. Mufalli and I cautiously surveyed our surroundings for any of the other multiple dogs that reside at the said residence, as we approached to assist Armida Jacobs. Patrolman J. Mufalli and I came to a confined hallway which led to the room where the attack was taking place. Patrolman J. Mufalli drew his weapon from his holster and fired several times at the animal. After Patrolman J. Mufalli terminated the threat, I quickly retrieved a medical bag from my patrol vehicle and went back into the residence where Patrolman J. Mufalli, Sergeant A. Urtasum of the Riverside Police Department and I began assisting Armida Jacobs with her injuries. During my travel to my patrol vehicle, I observed Sergeant A. Tilger rendering medical assistance to111111m h. Medical assistance was rendered to both victims prior to the arrival of the medical squad. Both victims were later transported to Cooper Hospital in Camden, N.J. for further medical assistance. *Field notes regarding this case exist and have been submitted as per agency policy. 4 CO P\i Supplemental Investigation Report Date: February 1, 2015 Department Case # New World Case # On the above date at 0000 hours, Burlington County Central Communications dispatched Delanco Police to 902 Hickory Street for a report of a dog attack. Upon my arrival, Ptl. Mufalli and Ptl. Welch were already inside the residence and I observed a male later identified as one of the victim's sitting on the front porch bleeding from his lower extremities. As I approached him, I observed through the residence's front bay window a large Rottweiler biting the leg of the homeowner Armida Jacobs. As a result, I rushed into the residence and as soon as I entered the foyer area I heard several gunshots. I then responded to the area of the living room where I observed Ptl. Welch and Ptl. Mufalli. At this time, I observed the Rottweiler lying motionless in the living room by the side of the injured victim Mrs. Jacobs. I then learned that Ptl. Mufalli had destroyed the Rottweiler by discharging several rounds from his duty firearm into the Rottweiler. After determining the threat Was over, I instructed both officers to render medical attention to Mrs. Jacobs and advised them that I would be responding to render medical attention to Mr. Jacobs. I then notified BCCC of the victims' injuries and requested they expedite BLS units to respond to our location. I then retrieved my medical supply bag and returned to Mr. Jacobs location. While meeting with Mr. Jacobs I observed that he had large puncture wounds and lacerations on both legs; both feet; and his upper left leg/rear end area. After making said observations, I immediately began treating his injuries. Shortly after, two BLS units from the Delran Township Emergency Squad did arrive on location and took over rendering medical attention to Mr. Jacobs. I did then respond to my marked patrol unit where I contacted BCCC and requested that they have Det. Russell contact me as well as the on call duty detective from the Burlington County Prosecutors Office. I additionally requested BCCC notify the Willingboro Township Police Department Shift Supervisor that the Willingboro Animal Control Officer was needed to respond to the scene to take custody of the deceased Rottweiler so that the dog could be tested for rabies and evidence be collected from the corpse. While waiting for said phone calls, both victims were transported to the Camden Medical Center, Camden NJ and the scene was secured. I was then contacted by Det. Russell. While communicating with him, I did apprise him of the incident and instructed him to respond to the scene to process the same. Moments later, I was contacted by Det. Thompson of the Burlington County Prosecutor's Office. While communicating with him I notified him of the incident. After providing him with the facts of the incident and notifying him that Ptl. Mufalli discharged his duty weapon, Det. Thompson advised that his office would not be responding to investigate the incident further. Det. Thompson instructed me to have this agency conduct their own investigation and contact his office should our agency need any further assistance. Printed 2/2/ :43:00 AM Page 1 of 2

5 in COPY I then met with Ptl. Mufalli and Ptl. Welch and released them from the scene. At this time, I instructed both officers to respond to police headquarters. Prior to leaving the scene, I did advise Ptl. Mufalli not to add any ammunition to the magazine that was seated in his duty firearm when he discharged his weapon and informed him that a BLS unit would meet him at police headquarters to conduct a medical evaluation on him. Ptl. Mufalli acknowledged the same and responded to police headquarters. Det. Russell did then arrive on location and I did provide him with the facts of the incident. Det. Russell then began processing the scene and while he was doing the same, I did meet with Debbie the Animal Control Officer. After getting permission from Det. Russell, I did escort Debbie to the deceased dog's location. Debbie did then retrieve the corpse and advised that she would be transporting it to the Mt. Laurel Animal Hospital where the evidence (projectiles) would be recovered. I then responded to police headquarters, where I met with Ptl. Mufalli. While on location, I did take custody of the magazine that was seated in his duty firearm when he discharged his weapon and observed that there were 8 rounds still loaded in the 15 round magazine. After photographing the magazine and 8 rounds, I did contact Det. Russell and notified him of the same. I then contacted the Cooper Medical Center to check on the status of the two victims. At this time, I was informed by staff that both were in the Intensive Care Units and that Mrs. Jacobs injuries were going to require surgery to be repaired but that the status of Mr. Jacobs was unknown. While communicating with the staff, I also confirmed that there were no projectiles fired from Ptl. Mufalli's firearm located in Mrs. Jacob. Tilger while at police headquarters did meet with Willingboro Township Animal Control Officer Debbie. While on location, she turned over the three projectiles that were recovered from the corpse of the Rottweiler that was terminated by Ptl. Mufalli. Debbi also advised that she had the head of the corpse and that it was going to be submitted for Rabies testing and that this agency would be advised of the results. I then logged the projectiles into evidence accordingly and completed a property impound form. The form was then forwarded to Det. Russell and a report was submitted. Tilger later informed me that he was advised by Mount Laurel Animal Hospital that they had removed three (3) projectiles from the deceased dog, leaving two (2) rounds unaccounted for.
