17 WLAOTC ANNUAL AWARDS APPLICATION FORM For Awards Year January 1, 2015 through December 31, 2015 You MUST apply for your awards nothing comes to you automatically. APPLICATION DEADLINE The APPLICATION must be received by TUESDAY FEBRUARY 16, Please submit a separate form for each dog. You may mail or hand deliver your application to the address below, or make arrangements to give it to Nancy Lovendosky at a class on Thursday, or Saturday. If you prefer, you may scan the COMPLETED APPLICATION and send it by , but you must submit information on the application form, not in the body of the . You do not need to submit your tray, if you have one, with the application, but trays must be received on or before the February membership meeting (FEBRUARY 16, 2016). If you miss this deadline, your tray will not be engraved. AWARDS ELIGIBILITY A total of six (6) credits must be earned during the Awards Year (January 1, 2015 through December 31, 2015) to qualify for awards. At least two (2) credits must be from service to the Club; the other four (4) credits may come from any combination of service or meeting attendance. For Juniors only, the requirement is two (2) credits, which may be from meetings or service. Title trophies will be awarded to any dog whose owner meets the above criteria. For other awards, where recognition goes to the dog/handler team, the dog must have been handled by the member (or household member) who is applying for the award(s). Awards must be earned during the calendar year unless otherwise indicated. Please do not include titles earned in previous years. 18 WLAOTC TITLE TROPHY Award No. 1 Dogs who earned Titles(s) or Certificate(s) where the designated initials may be used with the registered name will have these titles ENGRAVED ON A WLAOTC TROPHY OR TRAY. This includes but is not limited to titles in Obedience (eg., BN, CD, UCD), Agility (eg., NA, CL1-R, AD), Conformation (eg., CH, ALCH), Rally (eg., RN, RO1) and other performance sports (Herding, Carting, Tracking, Earthdog, Water Dog, Field, Draft, Flyball, Disc Dog, Freestyle, etc.). Certifications such as Canine Good Citizen, Temperament Tested, Canine Assisted Therapy, and the like, also qualify. This is a perpetual trophy. Titles earned in the future will be engraved on the trophy as long as the member has satisfied eligibility requirements for that awards year. The Club will purchase the trophy for the first award. If your dog has earned titles in previous years and you already have a tray, you will need to submit the tray to the awards committee. If this is the first year you are applying for an award for that dog, you only need to submit the application. 19 COMPETITIVE OBEDIENCE AWARDS Awards 2 through 16, 39 The Club also gives several awards to the dog/handler team who earned the highest scores in a particular category. If you completed a title during the awards year, you are eligible for the award even if you earned legs in previous year. Submit your scores even if you don t think you ll win. You may be surprised. A TROPHY OF PLAQUE WILL BE GIVEN FOR: 2. HIGH CD DOG Awarded to the dog that earned the three highest Novice legs for the first title CD or CD-equivalent title. 3. HIGH CDX DOG Awarded to the dog that earned the three highest Open legs for the first title CDX or CDX-equivalent title. 4. HIGH UD DOG Awarded to the dog that earned the three highest Utility legs for the first title UD or UD-equivalent title. 10. HIGH UDX DOG Awarded to the dog that earned the ten highest Utility Excellent legs, combining Utility and Open scores, for the first UDX or UDX-equivalent title. 39. HIGH OM or OGM DOG Awarded to the dog that earned the 200 points in Open B and Utility B as required by AKC for the first title OM or OGM-equivalent title. Rules Governing Award Nos. 2 through 4 and 10, 39. a. Scores must be from the same Registry (AKC, UKC, ASCA, etc). b. Legs do not have to be consecutive: failures before or between legs are permitted. c. Scores must be the three legs (or 10 legs for high UDX or all OM/OGM points) earned towards a title. Do not list scores earned after the title was earned. d. Awards must be applied for in the same trophy year as the final leg is earned; however, legs may be earned over several years. e. The dog is not eligible for this award if it has earned the equivalent title from a different registry. For example, a dog who earned an AKC CD may not later apply for the award using scores from a UKC CD. Applicants may only submit scores from the first title earned. 5. THREE HIGHEST LEGS (No failures) Awarded to the dog that earned the three highest legs towards any obedience title. There may be no failure prior to or between legs. Scores must be three legs earned towards a title. Do not use scores earned after the title was earned. Legs must be for the title earned in the current trophy year. 8. TOP OBEDIENCE DOG OF THE YEAR Awarded to the dog that earned the 5 highest scores in any Regular class or classes during the trophy year. Scores must be from the same registry (AKC, ASCA, etc.) but may be from different regular classes. 9. HIGH JUNIOR HANDLER Awarded to the handler with the 5 highest qualifying scores in Regular Classes. Jr. Handler must be under 18 years of age when scores were earned. If no Jr. Handler has earned 5 scores, the Jr. Handler with the highest number of qualifying scores will be awarded the trophy. Scores must be from the same registry (AKC, ASCA, etc.) but may be from different regular classes. 3

Scores must be from the same registry (AKC, ASCA, etc.). 38. HIGH BEGINNING NOVICE Awarded to the dog that earned the three highest trial scores in Beginning Novice. Scores must be from the same registry (AKC, ASCA, etc.). Dogs who have previously earned a CD are not eligible. 14. 25 AGILITY AWARDS A TROPHY OR PLAQUE WILL BE GIVEN FOR: 28. Any dog that wins a DESIGNATED PLACEMENT at the following events: a. USDAA Grand Prix Finals. b. AKC Nationals. c. AKC Invitational. d. NADAC Elite Nationals. e. ASCA Championships. f. CPE Nationals (Level 5 or C). g. Membership on the IFC or USDAA World Team. For Award Nos. 29, 30, 36 and 37, qualifying scores from AKC, ASCA, NADAC, CPE, and USDAA may be combined. Scores may only come from JWW/Jumpers classes and from the Standard/Regular classes. However, they may come from any combination of levels (Novice/Starters/Levels 2, Open/Advanced/Levels 3 or 4, or Excellent/Elite/Masters/Levels 5 or C levels, and from qualifying runs in Championship, Proficient, Skilled, Specialist, Preferred, Performance, Enthusiast or Veterans divisions. Scores from CPE Level 1 shall be counted for title trophy purposes only, and shall not count for the following special awards. 29.HIGH JUNIOR HANDLER Awarded to the junior with the 10 highest qualifying scores. Qualifying scores may come from any Standard or Jumpers class, any level, division, or venue, as set forth above. In case of a tie, full seconds under Standard Course Time (SCT) will serve as a tiebreaker. If no Junior Handler earns 10 qualifying scores, the Junior Handler with the highest number of qualifying scores will win the award, so long as the Junior Handler has at least 5 qualifying scores. Junior Handler must be under 18 years of age when the scores were earned. 30.THE DOUBLE Q AWARD Awarded to the dog who earns the largest number of Double Qs in Standard and Jumpers, any level, any division, any venue, as set forth above. The Standard and Jumpers legs must both be earned on the same day. 31. AKC DOG OF THE YEAR Awarded to the Top Agility Dog in AKC Masters classes with 10 points earned for every qualifying run and an additional point for every full second under SCT. 32.NADAC DOG OF THE YEAR The Top Agility Dog in NADAC Elite Chances, Jumpers, and Regular classes with 10 points earned for every zero-fault qualifying run and an additional point for every full second under SCT in Regular and Jumpers. 33. ASCA DOG OF THE YEAR Awarded to the Top Agility Dog in ASCA Elite Gamblers, Jumpers, and Regular classes with 10 points earned for every zero-fault qualifying run and an additional point for every full second under SCT in Regular and Jumpers. 9

USDAA DOG OF THE YEAR Awarded to the Top Agility Dog in USDAA Masters Standard, Jumpers, Gamblers, and Snooker classes with 10 points earned for every qualifying run and an additional 5 points for each 1st place, 3 points for each 2nd place, 2 points for each 3rd place, and 1 point for each 4th place. 35. CPE DOG OF THE YEAR Awarded to the Top Agility Dog in CPE Level 5/Level C Standard, Jumpers, Jackpot, Colors, Wildcard, FullHouse, and Snooker classes with 10 points earned for every qualifying run and an additional 5 points for each 1st place, 3 points for each 2nd place, 2 points for each 3rd place, and 1 point for each 4th place. A PIN OR MEDALLION WILL BE GIVEN TO: 36. AGILITY GROUP AWARDS Group awards will be offered for: a. Sporting b. Non-Sporting c. Toy d. Hound e. Working f. Herding g. Terrier h. All American/Other Breed Group awards will be given to the dog in each group that earns the highest number of points. Qualifying scores may come from any Standard or Jumpers class, any level, division or venue, as set forth above. Each qualifying score is worth 10 points. In addition, there will be a small premium for speed. One (1) point will be given for every full second under SCT in a maximum of 10 qualifying runs. The speed points must come from the highest level the dog has achieved during the awards year. (For example, if a dog has 12 qualifying runs in AKC Excellent and 12 qualifying runs in NADAC Elite, the applicant may choose which runs to include. If a dog has qualified six times in AKC Excellent and four times in USDAA Advanced and three times in NADAC Open, the six Excellent scores must be used for SCT points; the applicant may then add whichever Advanced or Open scores he/she wishes.) To win a group award, a dog must have earned a minimum of ten (10) qualifying scores during the awards year. A ROOKIE OF THE YEAR CERTIFICATE & PIN WILL BE GIVEN FOR: 37. AGILITY ROOKIE OF THE YEAR Awarded to the dog and handler who have not previously earned a title in any agility venue. Agility Rookie of the Year will be awarded to the dog/handler team who has acquired the highest level of titles during the Awards year. Titles may be earned in any Standard or Jumpers class, at any level (the first title of which, however, must be at the Starters/Novice/Level 2 level), division or venue, as set forth above.
