If you would like to volunteer, fill out the form below and return it with your entry form. Mark WORKER on the outside of the envelope. Returning this form will not guarantee entry into the trial, but we will take as many workers into slots that we can. Those with the least conflicts will be given preference to keep the trial flow smooth. Notification of worker status will be made as soon as possible by the Trial Chairman (swilson416@comcast.net). Notice to Exhibitors * Safety of the dogs is our primary concern. By entering this trial, exhibitors acknowledge they are familiar with the rules and regulations of this sport, and that their dogs are familiar with and able to perform all obstacles safely. * It is expressly understood that exhibitors alone are responsible for the behavior of their dogs and/or children. Any exhibitor whose dogs and/or children create unnecessary disturbances or engage in unsafe or disruptive behavior may, at the discretion of the Trial Committee, be asked to leave the Trial site. In such case, no refund of any fee paid will be made. Any incidents of dog aggression resulting in harm to other dogs or people that occur within the show grounds will be reported by the Show Committee to the proper authorities. Site Information Trial Hours: SAT/SUN - 6:00 am to 6:00 pm (EST) One Judge/One ring each day Ring is: 90' x 100' Indoor on artificial turf. Xpens and crates may be left over night, but the ACI will not be responsible for personal articles left at the site. No grooming is allowed in the building. No dogs may be left overnight and no overnight vehicle or RV parking will be allowed. * Entry fees will not be refunded in the event that a dog is absent, disqualified, excused or barred from the competition by action of the Trial Committee. * All dogs MUST be on-leash, crated, securely penned, or otherwise under physical control at all times when not competing or at the practice jump. Crating space is limited, please crate in the designated area. * There may be no more than 10 dogs per entry envelope. * No entry fee will be refunded if the trial cannot open or be completed by means of riot, civil disturbance, fire, or an act of God, emergency, act of public enemy, or any other cause beyond the control of the organizing committee. * Dogs are not required to be at the Trial until scheduled time of judging; however, exhibitors are responsible for being at the ringside when it is their turn to run. Judges need not wait for any dog holding up the class. Please check in with the Gate Steward. * Refunds: A refund of 100% of the entry fees will be given for any reason, if the request is received by the Trial Sec, in writing or ed, prior to closing. After closing and prior to trial starting, entry fees (less administrative, judging and AKC fees) will be refunded for: injured dogs, injured handlers, and bitches in season ( when a letter from your veterinarian is presented before 8:00 am the first day of the trial). A doctor/vet s letter may be required for injuries, at the discretion of the Trial Committee. Requests for refunds received after the start of the trial will not be given for any reason. ACI assumes no responsibility for any loss or injury sustained by exhibitors, handlers, nor any of their dogs, children, or personal property. All handlers must pick up after their dogs. Anyone witnessed not cleaning up after their dogs will be requested to leave the trial site and no refunds given. If you would like advance confirmation of your entry after the Draw date, please enclose a self- addressed and stamped (. 25) POSTCARD with your entry. No phone or confirmation will be given before the confirmation letter. * In accordance with AKC Rules Applying to Dog Shows, no dogs under the age of 6 months are allowed on the show site. All AKC rules apply regarding toys and food in the ring. Per Chapter 11, Sec 11 - only dogs eligible to be entered in an AKC trial are allowed on the trial grounds. There will be no food for sale in the building. Workers will be fed! There are several fast food restaurants in the area.

4 Be sure to check if the motel still accepts dogs and make your reservation early! Please have your canine companions on their best behavior so these motels will want our business again. We recommend dogs be crated in your rooms when you are not there. And, as always, pick up after your dog. There will be NO obstacle familiarization offered for Novice dogs. Electronic timing will be used in both rings. When downloading the premium from the website, send the ENTIRE sheet with the entry form on it, do not cut it in half. The AKC Agreement MUST be included. Prizes & Awards st th Rosettes and a choice of dog toy will be awarded for 1 through 4 place in each jump height in each class (dogs must qualify to receive a placement). Qualifying ribbons will be awarded to all dogs receiving a qualifying score. All prizes to be claimed the day of the Trial, none will be mailed. Regular [Preferred] JUMP HEIGHT DIVISIONS 8" [4"] for dogs 11" and under at the withers 12" [8"] for dogs 14" and under at the withers 16" [12"] for dogs 18" and under at the withers 20" [16"] for dogs 22" and under at the withers 24" [20"] for dogs over 22" at the withers 26" Dogs entered at owner s discretion Entries will be returned that are NOT complete! Owners are responsible for entering the proper height divisions. Handlers may opt to run their dog in a higher division (except the Preferred classes), but not in a division lower than the dog s proper height division. Dogs that run in a height division lower than their proper division in the Regular Classes will be disqualified. HEIGHT CARDS Effective January 1, 2001, all dogs competing in AKC agility must present an official AKC Jump Height Card prior to running in any class. All dogs without a permanent or temporary Height Card will have to be measured by the judge before running except 24" class dogs, unless a VMO or REP is present. ACI will accept actual cut- to- size, photocopies of the permanent or temporary Jump Height Cards with the entries for this show. However, all exhibitors should carry the original AKC official Jump Height Card with them to each trial, in order to present the card upon request. There WILL BE an official from AKC to measure dogs who do not have a height card. Times and additional details will be in the confirmation letter.

5 AKC Rules, Regulations, Policies and Guidelines are available on the American Kennel Club Web site, AGREEMENT I (we) agree that the club holding this event has the right to refuse this entry for cause which the club shall deem sufficient. In consideration of the acceptance of this entry and of the holding of this event and of the opportunity to have the dog judged and to win prizes, ribbons, or trophies, I (we) agree to hold the AKC, the event-giving club, their members, directors, governors, officers, agents, superintendents or event secretary and the owner and/or lessor of the premises and any provider of services that are necessary to hold this event and any employees or volunteers of the aforementioned parties, and any AKC approved judge, judging at this event, harmless from any claim for loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of this dog while in or about the event premises or grounds or near any entrance thereto, and I (we) personally assume all responsibility and liability for any such claim; and I (we) further agree to hold the aforementioned parties harmless from any claim for loss, injury or damage to this dog. Additionally, I (we) hereby assume the sole responsibility for and agree to indemnify, defend and save the aforementioned parties harmless from any and all loss and expense (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person or persons, including myself (ourselves), or on account of damage to property, arising out of or in consequence of my (our) participation in this event, however such, injuries, death or property damage may be caused, and whether or not the same may have been caused or may be alleged to have been caused by the negligence of the aforementioned parties or any of their employees, agents, or any other persons.
