2 2018 HUNT COUNTY JUNIOR LIVESTOCK SHOW ENTRY FORM Continued We, the youth exhibitor, parents or guardians and Ag Science Teacher or 4-H Club Mgr., hereby certify that we have read, understand, and will abide by all the rules and regulations in the current Hunt County Fair Livestock catalog. We further certify that we have not administered to this entry, nor has it received, to our knowledge, any substance not approved by the FDA and/or USDA for slaughter and/or breeding animals. This entry is not, nor will it be within any withdrawal time relative to the administration of any drug, chemical or feed additive approved by the FDA and/or USDA by the time the animal(s) is officially classified or checked in by the HCFLS. We, the undersigned, hereby consent and agree that the market and/or breeding animals(s) described on this entry application may, at the discretion of the HCFJLS management, be tested, evaluated, and/or analyzed for: 1) the improper use of substances not approved by the DAR and/or USDA; and/or 2) any method of altering or tampering with the natural conformation of any part of the animal s body. Improper usage or tampering will result in disqualification of the animal entry along with the exhibitor sanctions as established in the HCFLS catalog. All entry applications must be signed by the exhibitor, and parent or guardian if exhibitor is less than 18 years of age. All 4H members entries MUST have a signature from the Chapter leader certifying that the youth exhibitor has met all requirements to show in the HCFLS show. Incomplete signatures constitute no entry. We, the undersigned, hereby release, discharge and indemnify the Hunt County Fair Junior Livestock Show and the Hunt County Fair, its officers, directors and agents from all liability to the undersigned, and his or her personal representatives, assigns and heirs from any and all loss, damage, punitive and exemplary damages or any claims or demands including third parties, on account of injury to the person or property, including death of the undersigned whether caused by negligence of the releases, whether such negligence by sole, joint, concurrent or contractual, active or passive, strict liability or gross negligence or otherwise, while the undersigned is in any livestock area or participating in the livestock program and/or competing, observing, working in or for any purpose participating in the general livestock program of the HCFJLS. We, the undersigned further agree to indemnify and hold harmless Hunt County Fair, Hunt County Fair Junior Livestock Show, its officers, directors, agents for any loss, cost claim, judgment, or expense arising from any personal injury or death to any third party on account or occasioned by the negligent action of the undersigned, whether such negligence be sole, joint, concurrent, active passive, strict liability or gross negligence or the actions of any animals owned or in possession or care of the undersigned. We, the undersigned, have read and voluntarily sign this release and waiver for liability and indemnity agreement, and further agree that no oral representations, statements, or inducements apart from the above written agreement have been made. We, the undersigned, hereby certify this entry is eligible in accordance with the rules and regulations in the current Hunt Co. Fair Livestock catalog. All animals must leave the fairgrounds by 6:00pm Sunday, April 29, Any animals still on fairground property after that time will be forfeited to the Hunt County Fair Livestock Show. If amount of sale total and/or add-ons have not been collected from contributor when checks are issued. That amount will not be paid to exhibitor at that time. Once remainder is collected a check for the balance will be issued to exhibitor.
