(Rt 45) Ashtabula, Ohio Registration begins at Noon, Walk begins at 1:00pm Parking: Maple Ridge Golf Course About the Event: The 17th Annual Ashtabula County Boo-Wow walk is a spooky good time for both man and beast. The event features an enchanted walk along Maple Ridge Golf Course with 13 frightful stops for both you and your dog. Each stop includes a treat for you or your canine companion. After the walk, stay for some howling good food, costume contests with prizes, photos and more! Also don t forget to visit our awesome vendors booths! Registration fee is $25 per dog (includes event T-shirt and bag for trick-or-treating). Preregistration is encouraged. Day of registration is available however day of registrations and late registrations will NOT receive a T-shirt. Costume Contests: Prizes awarded in the following categories; Best Overall Costume, Best Group, Best Costume (Kids under 16), as well as prizes for the top three pledge collectors! Fundraising tips: This is a pledge walk fundraiser. Anyone is your potential sponsor! Ask those who you have daily contact with, relatives, friends, classmates, co-workers, neighbors, teachers, dog and cat lovers, anyone! Don t be afraid to ask. Tell your potential sponsors that you are raising funds to support the Ashtabula County APL! Be sure to collect your money up front, and to bring it to the event. Prizes are awarded for those who raise the most in pledges! About Maple Ridge Golf Course: Maple Ridge Golf Course is a beautifully landscaped rolling hills, partly wooded golf course. The walk is not on a permanent path, so be sure to wear the appropriate footwear for the time of year and weather conditions. The walk is not timed, and there are water and poop pick up stations along the way. The Ashtabula County Animal Protective League is a 501 (c) (3) membership based organization committed to providing temporary housing for the homeless dogs and cats of Ashtabula County, reducing pet overpopulation, and finding safe homes for adoptable dog and cats.

4 Boo Wow Walk Dog Rules For your dog s safety, we encourage the participation of dogs that are accustomed to regular exercise and are well adjusted to crowds of people, noisy environments, and other people as well as other dogs. Participating dogs must be fully vaccinated, as well as having an up to date rabies vaccination. All dogs must wear an I.D. tag. No dogs that are in heat. No aggressive dogs. No dogs under the age of six (6) months. Dogs must be leashed (6 foot maximum length) and under control. No retractable leashes! You must pick up after your dog. Encourage others to do the same. Supplies will be provided. A participant can have no more than two (2) dogs under his/her control. Children under the age of 12 may only handle one (1) dog with adult supervision. The Ashtabula County APL reserves the right to remove any dog from the walk that we feel is posing a threat to the safety of other people and/or dogs or that is physically unable to participate in the event. 5 th Annual Ashtabula County APL Boo-Wow Walk Liability Waiver The following must be signed in order to participate in the event, if under the age of 18, must be signed by a parent or guardian. I, (print name), voluntarily apply to participate in the th Annual Ashtabula County APL Boo-Wow Walk ( the walk ). I warrant that, to the best of my knowledge, that I am competent to walk safely and that I have no disability that would impair my ability to participate in the walk safely, or that would affect the safety of others. I have read the requirements set forth in official forms and warrant that any dog(s) that will be participating with me have met these requirements. I also warrant that any dog(s) participating with me have no health conditions or medical conditions that would impair their ability to complete the walk or that would affect the safety of others. I also understand that unanticipated events can and may occur. I recognize and accept all anticipated and unanticipated risks and hazards of participating in the walk involving a large number of people and dogs in a public area. I understand that the Ashtabula County APL makes no claims as to the safety of this route, or to the competence, physical condition, or character of the other participants. The walk will be conducted in a park open to the public at the time of the event and upon which the hazards of walking are expected. I will not hold the Ashtabula County APL liable for any consequences or its failure to detect any hazards, or to enforce any rule or condition in connection with the walk. I understand that the Ashtabula County APL is not responsible for the safety of me or my dog(s) walking with me in participation in the walk. In consideration of acceptance of the application to participate in the walk, I for myself, my heirs, executors, administrators, successors, assign and personal representatives, waive, release, and discharge all claims that I have, or that may accrue to me directly or indirectly as a result of my participation in the walk. I acknowledge and agree that this release applies to; all claims for injuries, damages, or losses to my person and property, real or personal (whether those injuries, damages, or losses to my person are known or unknown, foreseen or unforeseen, or patent or latent) including my death or personal disability, property damage, and injuries to any dog(s) participating with me. This release is intended to release, in advance, the Ashtabula County APL, their officers, directors, and sponsors, and any volunteers, employees, or agents and promoters involved in organizing or putting on the walk, as well as any private landowners and government entities whose property or jurisdiction is involved in the walk. I also agree this release applies to claims arising out of the carelessness or negligence of the persons or entities mentioned above.
