Guam is a rabies-free island, ranked second only to New Zealand by AQIS, the Australian Quarantine and Inspection Service, in worldwide protocol with regards to our quarantine procedures. Guam's quarantine law is designed to protect residents and pets from potentially serious health problems associated with the presence and spread of rabies. Success of the quarantine program is dependent on maintaining isolation of your pet from other animals for the required quarantine period. ABOUT US Harper Valley Kennels has been a licensed quarantine facility for over 28 years, and is currently the only civilian Animal Quarantine Facility (AQF) on the island of Guam. We are proud of our record of pet health and animal care. Our trained animal caretakers are concerned about the animals in their charge, and are available to help ease the transition to your new home in Guam. Please feel free to contact us by telephone, fax or to assist you with any additional information you may need. Our contact information may be found on the cover of this brochure. Our main purpose at our AQF is to board your pet(s) for the quarantine period, and observe them for signs and symptoms of rabies. In addition, the kennel facility is responsible for providing shelter and food for your pet(s). Only authorized persons are allowed to enter the quarantine facility. Included in this brochure is a list of rules and regulations that must be followed by any person who requests access to the compound. The AQF staff and the Government of Guam Department of Agriculture, Animal Quarantine Division will strictly enforce these rules and regulations. Each pet owner entering into a signed Agreement for Quarantine Services with our facility, and their co-owners, thereby acknowledge receiving said rules and regulations, and agree to abide by them. Determine If Your Pet Should Move to Guam Very old, very young (less than 9 weeks of age), chronically ill, or debilitated animals should not be shipped to quarantine. Pregnant animals, past 45 days gestation, are prohibited entry into quarantine. Owners with pets that experience difficulties during hot periods should use caution whenever shipping pets to Guam. High temperatures associated with high humidity usually reach their peak during the months of May through October. Vicious or intractable animals will not be accepted. If we are not told of this condition prior to your pet s arrival, we reserve the right to refuse entry to the quarantine facility. We need to be able to enter and depart freely inside your assigned kennel to feed, water, sanitize, and observe the behavior of your pet(s). Persons importing pets into the Island of Guam do so at their own risk. The AQF will not assume liability for any consequences for shipment or confinement. The owner assumes all liability by presenting the animal for quarantine. General Pre-Arrival Requirements The following requirements shall be met for all dogs and cats entering quarantine. HEALTH CERTIFICATE - A health certificate issued by an accredited veterinarian within 10 days prior to arrival is required. (Check with the specific airline regarding their health certificate time requirements.) The health certificate must be written in English, be an original document, facsimile or photocopy, bears a signature and legible name, address and telephone number of the certifying

3 veterinarian. The health certificate must also contain the following information: a. A complete description of your pet including age, markings, sex, breed and any additional identifying characteristics. b. Written declaration by issuing veterinarian that the animal was treated to kill all ticks and other external parasites within 14 days prior to arrival except when a veterinarian provides a written statement that such treatment may be detrimental to the animal's health. List the name of the treatment used and the date of treatment. c. Certification that your pet is free of any evidence of infectious or contagious disease. d. Certification by the issuing veterinarian to the accuracy of the information on the health certificate. e. Record of all required vaccinations. For rabies vaccinations, the name of the vaccine, lot/serial number, expiration date of the lot and date of vaccination must be included. To qualify for 30-day quarantine, list information on the two (2) most recent rabies vaccinations. f. The microchip number and date of implantation. ELECTRONIC MICROCHIP - All dogs and cats attempting to qualify for the 30-day quarantine program are required to have an implanted electronic microchip. The microchip must be U.S. made and readable with an AVID universal scanner (AVID chip, Home Again chip). It is the owners responsibility to have the microchip implanted by a veterinarian and ensures the microchip number is recorded on the blood sample submitted for the OIE- FAVN test. The microchip number must be included on the results of the test. VACCINATIONS All dogs and cats entering Guam are required to be vaccinated for common infectious agents. ENTRY PERMIT- Must be requested/obtained from the Department of Agriculture. The address is 192 Dairy Rd. Mangilao, Guam Tel:(671) /66 Fax (671) PLANNING YOUR PET S MOVE HOW MUCH QUARANTINE? 1. If your pet is not coming from a country recognized by Guam as rabies-free, then your pet will have to complete 120 days of commercial quarantine on Guam. This program is detailed later in this document. 2. The entry requirements must be completed prior to arrival and the animal may be allowed to leave the airport with you. Your veterinarian can help you with the pre-arrival requirements as detailed later. 3. If your pet is coming from a non-exempt country, you still may be able to reduce the duration of commercial quarantine potentially to as little as a single day, if you have the time and the resources to meet the requirements. This is the least expensive way to bring a dog or cat to Guam. Those people who have less than 120 days of preparation time can still endeavor to minimize the commercial quarantine duration on Guam. This can result in an individualized quarantine period on Guam that will be explained below. Pets that have not initiated the laboratory testing prior to arrival on Guam must begin the full 120 days of commercial quarantine. However, this period can be shortened if the FAVN testing and other requirements are completed during the commercial quarantine. It may be possible to release the animal into home quarantine for the remainder of the 120 days. 4. The other potentially significant difference regards the number of required rabies vaccinations. Animals participating in quarantines on Guam of less than 120 days will have had their rabies titers tested prior to arrival, and a minimum of TWO rabies vaccinations (lifetime) is required. The most recent rabies vaccination cannot be more than 365 days old, unless it is an approved 3-year vaccine. In that case the vaccine must have been administered not more than 36 months prior to arrival. Animals

4 participating in the full 120 day commercial quarantine on Guam will not require laboratory testing and must show proof of at least TWO completed rabies vaccinations (lifetime) prior to release from quarantine, and the most recent one must still be current at the time of release. There are more details on vaccinations below. THE HOME QUARANTINE OPTION: The entry requirements for the Home Quarantine option are, with a single exception, no different than those of any less-than 120 days quarantine. The pet still requires vaccinations, microchip, FAVN test, 4 etc. But if the FAVN titer meets or exceeds 1.0 I.U. (twice as high as the 0.5 I.U. required for the Calculated Quarantine), after the five days or less of commercial quarantine the pet can be transferred to home quarantine for the remainder of the 120 days of total quarantine. PRE-ARRIVAL STEPS COMMON TO ALL PETS For all quarantine programs, you must comply with each requirement and submit copies containing the veterinarian s printed and signed name on each document. Copies must be provided in English or with English translation. The owner, consignee, handler, or carrier shall pay all applicable expenses. This might include private veterinary examinations, vaccinations, microchip implantation, hospitalization, testing, medication, treatment, transportation and returned shipments to shipper or consignor for animals failing to meet prearrival requirements. 1. APPLICATION FOR ENTRY PERMIT Pet owners or their authorized representative(s) (authorization letter must accompany application) are responsible for submitting a completed application with the payment of a non-refundable fee of Sixty Dollars ($60.00) per animal permit, $5.00 per pet license per licensed year. Money orders or certified checks should be made payable to Treasurer of Guam. Please do not send cash. Puppies and kittens must be at least nine (9) weeks of age prior to any request for an entry permit. If an animal produces a litter while in quarantine, the offspring shall remain with the mother for the duration of her quarantine period, and additional fees may be charged by the commercial quarantine facility. 2. VACCINATIONS All dogs and cats entering Guam are required to be vaccinated against rabies and other common infectious diseases prior to release on Guam. This also applies to animals arriving from exempt areas. These vaccinations are best completed prior to arrival. A document attesting to the administering of the vaccinations must include the name and address of the pet owner, name and description of the animal (indicating its age, sex, breed, color, body weight, and markings), dates of vaccinations, (rabies certificates must include: manufacturer, serial/lot number, expiration date of the vaccine) and made certified with a name an legible signature/address of the veterinarian who administered the vaccinations. Health Certificates or other documents cannot be substituted for the actual Rabies Vaccination Certificates (copies are acceptable). For the other required vaccinations, the attending veterinarian must sign a letter or other document that details the date and types of vaccinations administered. a) Your pet requires certification of current rabies vaccination with an approved monovalent inactivated rabies vaccine administered not more than 365 days prior to the animal s release from quarantine on Guam (18 months if vaccinated with an approved 3-year vaccine). Do not vaccinate your pet for rabies after blood is drawn for the FAVN test (if performed). Also, the use of a modified live rabies vaccine is strongly discouraged and can require quarantine periods for as long as six months after that vaccination. b) Any subsequent rabies vaccination shall be administered not less than 30 days following the first

5 vaccination and not more than 365 days (36 months with an approved 3-year vaccine prior to release on Guam. All animals must have completed at least two rabies vaccinations prior to release from commercial quarantine. Other Dog Vaccinations Dogs shall be vaccinated not less than 10 days and not more than 365 days prior to arrival against: a. Canine Distemper Virus; b. Canine Infectious Hepatitis Virus or Canine Adenovirus-2; c. Canine Leptospirosis; d. Canine Parainfluenza; e. Canine Parvovirus; f. Canine Bordatella bronchiseptica (kennel cough) Other Cat Vaccinations Cats shall be vaccinated not less than 10 days and not more than 365 days prior to arrival against: a. Feline Panleukopenia Virus (feline viral enteritis); b. Feline Viral Rhinotracheitis (feline herpesvirus-1); c. Feline Calicivirus 3. AFFIDAVIT FOR EXPORT. Pet owners with animals originating directly from a designated rabies-free area must submit an affidavit for export or certificate of origin. The affidavit certifies that the dog or cat was not outside the area of origin during the 4-month period immediately preceding the date of shipment. In addition, the affidavit shall certify that the animal has not been vaccinated with a modified live rabies virus vaccine during the 4 months immediately prior to the date of shipment. Affidavits of this nature are generally issued by a government entity (i.e. state or national Department of Agriculture veterinarian or U.S. Army veterinarian) in the jurisdiction of origin. 4. QUARANTINE RESERVATION. Animals requiring quarantine on Guam must attach evidence of confirmed reservations at a government approved quarantine facility with the Entry Permit packet. It shall be the pet owner s responsibility to contact the facility directly for kennel reservation and rates. 5. HEALTH CERTIFICATE A health certificate issued by an accredited veterinarian within 10 days prior to arrival is required. (Check with the specific airline regarding their health certificate time requirements.) The health certificate must be written in English, and it must be an original document (facsimile or photocopy will be accepted for processing, however the original document shall accompany the animal). The health certificate must bear an original or stamped signature with legible name, address, and telephone number of the certifying veterinarian. The health certificate must contain all of the following information: a. A complete description of the animal, including age, markings, sex, breed, body weight, and any additional identifying characteristics. Mixed or similar wording is not acceptable for breed. Please indicate the nearest approximate breed. b. Veterinarian s statement certifying the following: 1) If determined to be so affected, the animal has been treated to remove internal (helminths and heartworms) and external parasites (fleas and ticks) within 14 days of arrival on Guam. 2) The date of any parasite treatment. 3) The name of the products used. The external treatment must use Fipronil or other long-acting product. ( Revolution is not acceptable). c. Certification that the animal is free of any evidence of infectious or contagious disease (to include heartworm testing/treatment information).

6 d. Certification by the issuing veterinarian to the accuracy of the information stated on the health certificate. e. The microchip number and the date of implantation (not required for 120-day Commercial Quarantine). f. The name, serial or lot number, expiration date of the lot, and date of rabies vaccination must appear on the health certificate. ADDITIONAL REQUIREMENTS FOR CALCULATED & HOME QUARANTINE 1. ELECTRONIC MICROCHIP All dogs and cats attempting to qualify for something other than the full 120-day commercial quarantine program are required to have an implanted electronic microchip. The microchip must be U. S. made and readable with an AVID universal scanner (i.e. AVID chip, Home Again chip). 2. It is the owner s responsibility to have the microchip implanted by a veterinarian and ensure that the microchip number is recorded on the serum sample before it is submitted for the rabies testing. The microchip number must be included on the results of the rabies antibody test. 3. OIE-FLUORESCENT ANTIBODY VIRUS NEUTRALIZATION TEST (aka OIE-FAVN Test, Rabies Antibody Test). A rabies blood test (requiring 1 to 3 ml of serum) to determine if dogs and cats have responded adequately to the rabies vaccination is required prior to arrival as the first step to qualifying for the 0-5 day quarantine program. The testing laboratory must submit the test results directly to the Animal Quarantine Program at the Department of Agriculture, 163 Dairy Road; Mangilao, Guam a) The OIE-FAVN test must be conducted no more than 12 months prior to arrival into Guam. A result of 0.5 I.U. of rabies antibodies or greater per milliliter of blood or is required (1.0 I.U. required for the Home Quarantine). When updating rabies vaccinations prior to conducting the FAVN testing, you should wait at least 10 days before drawing the blood sample. This gives time for the pet to respond to the vaccine and build a titer high enough to pass the FAVN testing requirements. NOTE: DO NOT ADMINISTER FURTHER RABIES VACCINATIONS AFTER THE BLOOD IS DRAWN FOR THE FAVN TEST. b) The day that the laboratory receives the OIE-FAVN sample counts as the first day for the 120- day countdown. c) To be considered valid, the test results must include the number of the implanted microchip and a complete description of the animal (sex, breed, age, color or color pattern, any visible unique identifying characteristics, etc.) d) If your pet passes the pre-arrival OIE-FAVN test and provided that all other requirements are met at least 120 days prior to arrival, your pet will then be considered as having met the prequalification requirements for the Calculated Quarantine (Home Quarantine requires higher titer). No quarantine of less than 5 days can be guaranteed based on the availability of a veterinarian to perform the arrival quarantine examination. Note: If documentation is incorrect or incomplete, arriving animals will be sent to a commercial quarantine facility while awaiting proper documentation. The costs for this holding period will be born by the owner. If documentation is incomplete to meet the requirements a less than 120-day quarantine program, the full 120-day commercial quarantine program will go into effect.

7 POST-ARRIVAL REQUIREMENTS Post-Arrival Health Examination a) Any dog or cat eligible for less than 120 days of quarantine shall be transported directly to a commercial quarantine facility where a post-arrival examination by the Territorial Veterinarian or a licensed veterinarian approved by the Director must be completed. This same examination must be completed for all dogs and cats arriving from EXEMPT AREAS too, but that examination shall be arranged and completed by the owner/agent within 72- hours of pet arrival, and any veterinarian licensed on Guam can complete this examination. No animal can be sold until this examination and any required treatments have been completed. Failure to comply can result in the full 120-day quarantine at the owner s expense. b) The veterinarian responsible for the post-examination must submit a health examination certificate to the department verifying the health of the dog or the cat and provide his/her recommendations for the animal s early release or continued quarantine status as deemed necessary. The examination certificate along with the attending veterinarian s recommendation must be submitted to the Department of Agriculture prior to approval and release of any dog or cat. A certificate of completion must be obtained from the department prior to release. c) The post-arrival examining veterinarian may require quarantine for observation for animals that show signs of illness or disease during the examination. * Animals eligible for Calculated or Home Quarantines that are held by the examining veterinarian during post-arrival examination due to any health anomalies must be cleared for release by the Territorial Veterinarian or a licensed veterinarian appointed by the Director. Quarantine issues not specifically addressed by this document will be resolved by the Director or the Territorial Veterinarian. Shipping Procedures Pets may only enter Guam through the Guam International Airport. You must contact the AQF within two (2) days of your pet s arrival to ensure inspection and clearance is conducted within 72 hours by the Customs and Quarantine Agency. Upon arrival into Guam, airline personnel will transport all dogs and cats directly to the Customs and Quarantine area at air cargo (yellow building) pending inspection and clearance. At the appointed time, the AQF representative will meet with an officer of the Customs and Quarantine Agency. A Customs, Agriculture & Quarantine Inspection Service Charge of $5.00 per single consignment will be charged to your account in the event you are unable to make arrangements in advance to pay for this fee. In addition, based upon the arrival time of your pet, the Customs and Quarantine Agency may assess a Customs Carrier Off-Duty Service Charge. This charge varies according to the assigned Officer s wage, and which payment is also the responsibility of the pet owner. Required pet documents (the Original Rabies Certificate, Original Health Certificate, Immunization Record must be attached to your pets travel kennel/carrier) will be reviewed and, if complete, your pet may be cleared by Customs and Quarantine. Your pet will then be transported to Harper Valley Kennels in Chalan Pago by the AQF representative. You do not have to be present when your pet arrives. Label your pet's airline crate with the full names of owners and co-owners. Include complete local addresses and phone numbers to be used in case of emergency. To facilitate clearance of your pet, it is suggested that an extra set of required pet documents be placed in an envelope and securely attached onto your pet s carrier in the event other copies are lost en route. Note that only the name appearing on the shipping documents (Shipmaster's Declaration) will be recognized as legal owners of your pet.

8 Airline Animal Carriers The airline carrier that your pet arrives in will be kept at the AQF for the duration of the quarantine period. In the event of a typhoon, they are used to transport your pet to typhoon safe structures where they will remain until any typhoon threat has passed. Carriers are allowed to remain in the kennel upon owner's request. The AQF will store any carriers that do not remain inside the kennel. We are not responsible for any carrier left at the Facility. KENNEL ACCOMODATIONS There are three different kennels for dogs: Premium, Deluxe and Standard. We currently have three Premium kennels available which are very spacious, approximately 9 feet wide by 15 feet long. The Deluxe kennels are approximately 15 ½ feet long by 4 ½ feet wide, and the standard kennel measuring 8 feet long by 4 1/2 feet wide. Deluxe and standard kennels are assigned according to the size of the pet. The premium kennels are available to larger dogs by request, at an additional fee. Kennels are assigned on a first come basis. Our cat kennels are fully contained within a safety perimeter, and are available in only one size: Deluxe. The Deluxe kennels are approximately 15 ½ feet long by 4 ½ feet wide. Only one pet is allowed in each kennel for the 30 day or the 5-day or less program. Multiple pets in the same kennel during the 120 day program is available. When you make your reservation, you may request that your dogs be placed in either adjacent or facing kennels. Since our cat kennels are completely confined, there is no such accommodation. Harper Valley Kennel assumes no responsibility for any items such as transport carriers, bedding, collars, toys, leashes, etc. Owners are requested to remove such articles from the Facility upon the release of their pet. Items that are difficult to disinfect or that create a safety hazard for employees, such as wet bedding, rubber floor mats with grooves or holes, foam mats, large platform beds, chaise lounges and chairs, etc. may be removed from kennels by quarantine facility personnel. CARE OF YOUR PET Co-Owners Listed owners (from Shipmaster's Declaration) and designated co-owners are responsible for shipping, health care, fee payment and release at the end of the quarantine period. Co-owners have the same responsibilities and duties as owners and can act in the owners behalf. Co-owners must obtain a written authorization from the Listed Owner and provide it to the AQF office. Registered visitors do not have the authority to act on behalf of the owner. Animal Medical Care You are responsible for your pet's health while in quarantine. When visiting, it is helpful to check for ticks (dogs), skin or ear problems, or any other abnormality which may indicate a potential medical concern. Animal caretakers will make every attempt to alert the Owner or Co-owner of any noticeable medical problems. The AQF veterinarian will also be contacted, and provide any medical care necessary only if

9 the Owner/Co-owner authorizes treatment, except in emergency situations. If you believe a problem exists, you may request in writing for a veterinary check by the AQF veterinarian: All examination request submitted will be attended to the following day (except in emergencies). Provide all owner and pet information and describe the condition you wish checked as clearly as possible. For minor problems, treatment may be initiated following examination unless you specify otherwise. Results of the exam and associated charges, if any, can be obtained by visiting the AQF office during afternoon visiting hours or by telephoning the office at (671) Medication At your request, the AQF veterinarian will administer your pet s vaccinations. Questions regarding pet medications can be directed to the veterinary staff. In general, nonprescription medications, such as vitamins, nutritional supplements, etc., are not administered by facility employees unless specifically requested by Owner. A daily fee of $3.00 per pet will be charged for any medication or supplement dispensing. If you provide medication for your pet, it must be in a plastic bottle. Pills must be sized to the proper dosage. Please label medication containers with your last name, pet's entry and kennel number. Feeding instructions, changes in feeding instructions, food, medications and medication instructions must be submitted with precise written instructions on dispensing. Please notify the AQF staff if you administer any medications to your pet. Protect Your Pet from Heartworm Heartworm disease is a serious consequence for both dogs and cats not on monthly preventive while in Guam. Dog and cat owners are required to have their pet tested prior to entry into Guam. We require that owners administer monthly heartworm medication themselves. At the owner s request, the AQF will administer heartworm preventive upon receipt of medication. Please provide a supply of a monthly heartworm preventive medication (such as Interceptor or Heartgard) to protect your pet. Heartworm medication is given by facility personnel the first day of the month for simplicity. You may purchase the medication directly at the AQF office. Tick Control Ticks are a year-round problem in Guam. We recommend that you begin treating your animal with an appropriate treatment of your choice prior to your pet s arrival to the quarantine. The Kennel personnel will spray bi-weekly for these parasites. However, you as the pet owner are responsible for the maintenance of treatment to your pet for ticks and fleas. Tick control products are required to prevent your pet from getting ticks. Two products work well to prevent ticks in Guam. The best product is Frontline containing Fipronil. Frontline is applied to small dogs over 8 weeks of age. Frontline can be used for larger dogs over 10 weeks of age. You can request for this product from the kennel personnel (this is not provided by the kennel and will be an additional charge). Cats generally do not get ticks and do not need tick control products.

10 Feeding Your Pet Unless otherwise requested, your pet will be fed the food provided by the kennel facility. and will be dispensed utilizing the age-range formulated by the manufacturer. Food consumption is closely monitored. You will be notified by an AQF caretaker if your pet has problems with the diet during quarantine. If you prefer to feed your pet another product, or if your pet requires a special diet, you are required to supply the food and feeding instructions. Enough food must be provided for at least one month s feedings, and must be monitored by the owner. In the event the food supply is exhausted, a penalty of $20.00 will be assessed. No adjustment of quarantine fees is made for those supplying special food to their animals. Food containers must be individually labeled with your last name in legible ink. If you are supplying dry food, it must be submitted in a secure vermin-proof heavy plastic or metal container, with a tightly fitting cover, and be large enough to hold the entire amount of food supplied. Grooming Owners are responsible for grooming their own pets. Our facility provides a grooming area, so please make your grooming appointments early. Appointments, scheduled in ½ hour increments, must be reserved at least two days in advance. You may reserve one grooming appointment per week, per pet. Grooming hours are during visitation. The grooming area contains a basin for cats and small dogs, and a standard tub for larger pets. Owners must bring all grooming supplies, including towels. A caretaker will accompany you with your dog, with a properly fitting collar and leash, to the grooming facility. Cats will be transported to the grooming facility in their cat carrier. You may bathe your dog within the kennel at any time during visiting hours. Cats, however, must be bathed within the grooming facility inside a basin. Owners are responsible for the proper handling of pets during the grooming period and when walking to and from the grooming facility, and must be accompanied by an AQF caretaker. In addition, owners are expected to maintain the grooming facility in a tidy condition. Revocation of grooming privileges may result if animals are not handled in accordance with facility rules or basins are left dirty. RULES AND REGULATIONS The Primary purpose of quarantine is to observe your pet(s) for signs and symptoms of rabies and prevent the virus introduction into Guam. There are some unexpected situations that will require temporary closing of the kennel for the safety of you and your pet. If this does occur, there will be additional charges for the kennel personnel to provide services for your pet that are normally provided by you (baths, dips, and administration of medications, etc.) Current quarantine fees are based upon the owners providing their pets with the services mentioned above. You will be provided with a current visitation schedule upon arrival of your pet. Pet owners must visit their pet at least once a week to observe changes in pet behavior (i.e. eating habits, signs of sickness, etc). If you are unable to visit the pet (i.e. pet shipped prior to your arrival), then an emergency contact number shall be provided, along with an emergency treatment authorization form signed,

11 authorizing our on-site veterinarian to provide treatment in the event of a medical emergency. If any changes are noticed during your visit with your pet, you must immediately alert the AQF staff. Any treatment or action taken, including veterinarian treatment, is the pet owner s decision. You may request treatment with our on-site veterinarian, or make arrangements with your registered off-site veterinarian to see your pet at the AQF. The AQF veterinarian will only take action if an emergency situation arises and we are unable to reach you, or an assigned co-owner. Visiting your pet during the quarantine period is a privilege. It may be denied if the rules and regulations are not followed and/or payments are delinquent. If your visitation privilege is denied, you will be charged additional fees for the maintenance of your pet. The following is a list of rules that owners and visitors are obligated to comply with while on the quarantine facility grounds, or risk forfeiture of their visiting privileges and/or notification to the Government of Guam Department of Agriculture, Animal Quarantine Division, thereby risking exportation of your animal. Authorized visitors must be 18 years or older. Minors are allowed to visit while accompanied by owner. When entering the facility, please sign in at the gate, and proceed directly to your kennels to avoid disturbing other pets. All owners must visit their pet(s) inside the pet s kennel with the door closed. We are required by law to report to the Department of Agriculture any pet that has come out of their assigned kennel for any reason. Visiting, petting and feeding other pets is strictly prohibited. No one will be allowed to visit your pet without your registered permission on file. For the health and safety of all persons and animals, all kennel doors must be properly latched and doors completely closed in order to prevent escapes and injuries. Walking or removing dogs from kennels is prohibited, except taking shortest distance to and from grooming facility. Dogs must be collared and leashed properly. Any removal of an animal from its kennel must be done by an AQF caretaker ONLY. Children are welcome in the AQF only in the company of an authorized adult who shall be responsible for the minor's behavior and safety. However, they must remain inside the pet s kennel at all times. Please restrict children from running, playing, behaving boisterously or putting fingers through the wire mesh, This is for their safety as well as that of all of the animals. Skateboards, skates of any type, bicycles, etc. are not allowed on the facility grounds. The quarantine facility is a family-type environment. Please wear appropriate clothing while on the facility grounds. In addition, any person using profane language will be required to leave the premises immediately, and will be denied any future visitation privileges. No other animals are allowed on the quarantine facility premises. No alcoholic beverages of any kind are allowed on the premises. If you violate this rule, you will be denied visitation privileges, and be reported to the Department of Agriculture, Animal Quarantine Division, thereby risking the exportation of your animal. Medication Dispensing fee $ 3.00 per day Bathing $ per Pet, depending on weight. Late payment fee 10% of past due amount All quarantine fees must be paid before the release of the pet.

If any payments are received in the form of a check, it will be immediately returned to the maker, and a finance charge for late payment may be assessed.. Note: It is the Owner s responsibility to maintain copies of all documentation for each pet. An original signed Agreement will be provided by the Kennel. Any additional copies will be provided at a cost of $15.00 per copy.
